Requirements
An active Licensed Practical Nurse state license is required.
A valid driver's license is required.
Two or more years of applicable nursing experience are required.
Responsibilities
Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes, including conducting patient home visits.
Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's needs.
Manage emergency situations in a prompt, precise, and professional manner.
Complete medication administration and treatment needs for all assigned patients.
Provide nursing care in accordance with applicable regulations and company policy.
Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
Desired Qualifications
Bilingual Spanish language ability is preferred.

Responsibilities
