Requirements
  • Three to five years of progressively more responsible experience as a registered nurse in a clinical setting is preferred, with expertise in geriatrics substantiated through education, experience, and credentials.
  • At least one year of experience with a frail or elderly population is required.
  • Maintain a current Michigan Nurse Practitioner license in good standing and fulfill continuing medical education requirements.
  • Comply with PACE guidelines for tuberculosis screenings, infection control, basic life support, and mandatory in-service requirements.
  • Establish and maintain interpersonal and interdepartmental relationships.
  • Apply principles of teaching and learning theories when planning and implementing educational activities.
Responsibilities
  • Provide primary care to an assigned panel of PACE participants under the direction and supervision of the center Physician-In-Charge.
  • Perform monthly quotas of comprehensive annual and semi-annual assessments.
  • Conduct chronic care management visits every three months.
  • Update management and treatment plans in response to changes in chronic disease or new diagnoses.
  • Confer with the physician and other clinical team members regarding changes in participant status.
  • Prescribe pharmacy medications, including delegated Schedule III through Schedule V controlled substances, under supervising physician delegation.
  • Make specialty and diagnostic referrals with staff physician guidance.
  • Initiate appropriate diagnostic studies, including laboratory, X-ray, and other specialty procedures, with physician approval or oversight.
  • Coordinate care with the interdisciplinary team, patient care team, utilization management, and other PACE services, communicating participant changes to the interdisciplinary team.
  • Provide preventive care services and acute care management and treatment.
  • Educate participants.
  • Document in the medical record using the SOAP format, complete notes with a full assessment preferably within 24 hours and no later than 72 hours, and facilitate capture of hierarchical chronic conditions coding.
  • Contribute to utilization management.
  • Facilitate advance directives, goals-of-care discussions, and end-of-life discussions with participants and families.
  • Manage end-of-life care with support from a consulting hospice service.
  • Provide care in the clinic, participants' homes, nursing facilities, and through telemedicine.
  • Participate in rotating after-hours medical assistance through telephone triage or after-hours home visits as needed.
  • Participate in the monthly education conference.
  • Practice within the Nurse Practitioner's scope under Michigan law and obtain physician supervision or delegation when required.
  • Serve as a role model for professional nursing practice among peers.
  • Support and contribute to the development of nursing students and students in other health-related disciplines.
