About the job
- Graduation from a college or university accredited by the Accreditation Council on Pharmaceutical Education.
- One year of hospital-based pharmacy experience after graduation in unit-dose distribution and intravenous additives and/or completion of a hospital residency.
- An active, unrestricted Pharmacist license.
- Basic competency in Microsoft Word, Microsoft Excel, and Microsoft PowerPoint, with the ability to type 40 words per minute.
- BLS and ACLS certifications.
- Ability to pass a government background check and obtain a government security clearance.
- Precept pharmacy students and residents in a non-supervisory role when present.
- Ensure work areas are clean and safe according to applicable regulations.
- Ensure compliance with USP 797 and USP 800 regulations, including proper barrier or protective clothing, beyond-use dating, and aseptic technique.
- Ensure compliance with The Joint Commission regulations throughout the facility.
- Manage patient-specific pharmacokinetic regimens, including vancomycin, anticoagulants, and aminoglycosides.
- Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
- Provide medication and health-care-related in-service training to clinical staff on new medications, appropriate medication use, comparisons of current medication therapies, and other department-approved topics.
- Provide clinical direction to pharmacy personnel.
- Consult with patient-care teams through multidisciplinary rounds and antimicrobial-stewardship monitoring to support evidence-based patient care, and document pharmacy interventions in the electronic health record.
- Document significant adverse medication events with prescribers, in the patient's medical record, and in the Patient Safety Reporting system.
- Document and report patient-safety concerns using the Patient Safety Reporting system.
- Facilitate medical-staff drug-utilization reviews as needed.
- Assess patients' responses to drug therapy and plan drug therapy based on physician-established diagnoses.
- Order and evaluate laboratory tests needed to evaluate drug-therapy effects and outcomes.
- Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states in cooperation with medical staff.
- Monitor and manage pharmacotherapy requiring periodic adjustment because of specific or changing pharmacokinetic characteristics.
- Administer prescription or non-prescription drugs according to established agreements or military treatment facility protocols.
- Evaluate medical and medication histories for drug-related problems and adjust drug therapy accordingly.
- Consult with physicians, dietitians, nurses, physical therapists, and other healthcare providers about patient treatment needs or options.
- Perform the full range of pharmacist procedures.
- Review patient profiles at dispensing to closely monitor medication therapy.
- Verify prescription directions, dosage, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data for individual patients.
- Ensure the accuracy of pharmaceutical products prepared or processed by technical staff.
- Consult with specialty practitioners to provide optimal patient pharmaceutical care.
- Document significant medication interactions and pharmacy interactions with prescribers, and conduct adverse drug reaction reporting in accordance with applicable accreditation standards.
- Assist the Outpatient Pharmacy.
