Requirements

Responsibilities
  • Accept client admissions as assigned by the agency.
  • Complete initial home assessments of clients, caregivers, and home environments for admission and continued services to determine eligibility and needs according to agency regulations.
  • Prepare written plans of care for administering care to clients.
  • Conduct supervisory visits to clients' homes to ensure the quality and appropriateness of services and the health, safety, and welfare of clients.
  • Provide necessary supervision to aides and clients and record significant documentation in clients' files.
  • Observe, evaluate, and document the adequacy and appropriateness of care based on recipients' functional status and medical and social needs.
  • Submit documentation requesting increases or decreases in members' hours of care.
  • Discuss clients' and families' satisfaction with services during scheduled supervisory visits.
  • Review aide records during supervisory visits in accordance with regulatory and organizational policies.
  • Maintain timely and comprehensive clinical records, reports, and agency guidelines.
  • Correct documentation as requested by the Clinical Supervisor.
  • Coordinate care for clients with staffing coordinators and outside agencies or organizations as needed.
  • Arrange referrals to physicians, skilled services, and community agencies in collaboration with other staff disciplines as needed.
  • Maintain ongoing communication with paraprofessional staff regarding changes in client conditions or plans of care and evaluate effectiveness.
  • Advocate for clients and caregivers and report issues, concerns, or needed changes to the agency or support staff.
  • Participate in clinical meetings and complete in-service training as directed.
  • Maintain a schedule for required visits and complete visits according to regulatory and organizational criteria.
  • Submit monthly Level of Care reviews according to applicable guidelines.
