  • Ability to communicate effectively with patients regarding their care.
  • A bachelor’s degree in a healthcare-related field or an equivalent combination of education and experience.
  • No experience is required.
  • The position requires constant standing and walking, frequent oral communication, hearing, lifting/carrying and pushing/pulling objects weighing 0–25 pounds, reaching, gripping, and keyboard use/data entry, and occasional physical movements including bending, stooping, crawling, kneeling, sitting, squatting, repetitive foot/leg and hand/arm movements, lifting/carrying and pushing/pulling objects weighing 25–50 pounds, lifting/moving patients, and driving.
  • No professional license or certification is required.
Responsibilities
  • Review charts to identify ongoing health maintenance needs and facilitate next steps to assist patients in meeting those needs.
  • Obtain outside medical records and assist with uploading them into the electronic medical record.
  • Schedule patient appointments and assist licensed personnel to enhance comprehensive patient care and reduce gaps in care.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
