Requirements
  • A Master's degree in Psychology, Counseling, Social Work, or a related field.
  • An active and unrestricted license in the respective state, such as Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Marriage and Family Therapist, or Licensed Psychologist.
  • Strong clinical assessment, diagnostic, and treatment planning skills.
  • Knowledge of mental health conditions common in skilled nursing facility populations, including depression, anxiety, dementia, and adjustment disorders.
  • Proficiency in evidence-based therapeutic approaches, such as cognitive behavioral therapy, trauma-informed care, and grief counseling.
  • Ability to work collaboratively within an interdisciplinary team.
  • Strong organizational, time management, and documentation skills.
Responsibilities
  • Conduct comprehensive psychosocial assessments for residents upon admission and as needed.
  • Develop, implement, and monitor individualized treatment plans.
  • Provide individual, group, and/or family therapy sessions.
  • Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
  • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
  • Collaborate with the clinical team, attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care.
  • Participate in care plan meetings and interdisciplinary team discussions.
  • Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations.
  • Provide crisis intervention and behavioral management strategies when needed.
  • Educate staff and families on mental health conditions and effective communication strategies.
  • Ensure compliance with the Health Insurance Portability and Accountability Act and ethical standards of practice.
Desired Qualifications
  • Two years of clinical experience, ideally in geriatric, long-term care, or medical settings.
  • Familiarity with electronic health record systems.
  • Excellent communication and interpersonal skills with residents, families, and staff.
