Requirements
  • At least one year of registered nurse experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver's license are required.
  • Knowledge of terminally ill patients and their families, hospice care, hospice-service disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC is required.
  • Ability to perform patient assessments, apply hospice knowledge, communicate effectively, document clinical information, operate nursing assessment tools and office equipment, and maintain transportation for work is required.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients' homes, assisted living facilities, or other care settings.
  • Perform comprehensive initial, ongoing, in-person, telephonic, and reassessment screenings, and provide education to patients, families, and caregivers.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Coordinate care with physicians, nurse practitioners, LPNs, CNAs, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, bereavement guidance, and caregiver training.
  • Educate patients and caregivers about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, hospice philosophy, symptom and pain management, and end-of-life expectations.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care physicians, and hospice physicians.
  • Support home health, palliative, and hospice referrals and transitions between levels of care.
  • Document screening and assessment findings, physician orders, progress notes, care plans, and other clinical records in a timely manner.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient's care plan requires participation.
  • Participate in on-call and patient-support activities and conduct on-call services in a clinically competent and responsive manner.
  • Follow infection-control, regulatory, compliance, and organizational policy standards.
  • Participate in quality-improvement, scheduling, and interdisciplinary-group activities.
