Requirements
  • Prior experience as a Certified Nursing Assistant, preferably with people with complex mental health needs.
  • A high school diploma or equivalent is required.
  • A valid driver's license is required; drivers using a personal vehicle must maintain adequate insurance and a safe driving record.
  • Incumbents must be at least 21 years of age, have maintained a valid United States driver's license for at least one year, and pass a driver's screening background check.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, operate a computer and office equipment, assist clients with daily living tasks, and communicate with clients, staff, and external representatives.
  • Current Certified Nursing Assistant certification in the state of service is required.
  • CPR certification is required within two weeks of hire, First Aid certification within two weeks of hire, Medication Administration Program training within 150 days of hire, and CPI training within 60 days of hire.
  • Ability to work effectively and collaboratively with colleagues and communicate and document nursing care accurately.
  • Ability to work professionally and confidentially, balance competing priorities, make independent judgments and decisions, and use personal computer applications and equipment.
Responsibilities
  • Provide assistance with activities of daily living according to each individual's treatment plan, including community membership, community relationships, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range of motion exercises.
  • Monitor and observe individuals served for changes in physical, mental, or emotional status.
  • Assist with self-administration of medication in accordance with Vinfen policies and procedures and provide for individuals' nutritional needs.
  • Transport, assist, and accompany individuals served to medical, dental, and other appointments as directed.
  • Create and maintain safe, clean, welcoming, and respectful individual rooms and common areas that meet safety and regulatory standards.
  • Document nursing care and services provided, including daily shift and TCM notes, and keep medical records and confidential files current and accurate.
  • Follow policies and procedures for incident reporting and medication errors and ensure paperwork meets organizational and external-agency standards.
  • Report changes in clients' status to managers or administrators and communicate with colleagues, staff, families, and guardians.
  • Use and safely maintain assistive and medical equipment, including wheelchairs, walkers, Hoyer lifts, glucometers, nebulizers, and oxygen equipment.
  • Provide opportunities for individuals served to participate in home, program, and community events while respecting their preferences and choices.
  • Advocate for individuals served and develop effective relationships with individuals, families, guardians, and other significant contacts.
  • Act as a liaison and collaborate with internal departments, outside agencies, and community providers.
  • Attend and actively participate in staff, training, and community meetings.
  • Act as a positive role model for individuals served and assist with orienting new staff.
  • Adhere to Vinfen policies and procedures and perform other related duties as assigned.
