Requirements
  • A bachelor's degree in social work or a related field is required.
  • Proficiency with Microsoft 365 and knowledge of Connections are required.
  • Ability to perform the essential functions of the job with or without a reasonable accommodation is required.
  • Ability to handle and resolve recurring problems is required.
Responsibilities
  • Monitor child safety and well-being through casework contacts and home visits with children, parents, and foster parents.
  • Co-develop, coordinate, and monitor service plans in alignment with family needs.
  • Collaborate with the interdisciplinary team to ensure children’s medical, mental health, academic, and, where applicable, housing and career goals are being achieved.
  • Document all activities as progress notes in Connections within program-defined timeframes.
  • Participate in Family Team Conferences and complete FASPs in accordance with program deadlines.
  • Attend Family Court hearings for assigned cases and provide written and verbal testimony about the family’s progress toward service-planning goals.
  • Complete court reports and Permanency Hearing reports in accordance with program guidelines.
  • Facilitate in-person family-time visitations and phone and video contact to support frequent, positive, and consistent contact between children and their parents.
  • Proactively address barriers to reunification and other permanency options.
  • Provide meaningful and responsive support to families in times of need.
  • Participate in treatment-planning efforts and Trauma System Therapy team meetings as appropriate.
  • Participate in job development and professional development activities.
  • Perform other duties and responsibilities as assigned within the scope of the position.
Desired Qualifications
  • Prior child welfare experience is preferred.
