Requirements
  • Consecutive employment in a position with comparable responsibilities within the past five years.
  • A minimum of two years of certification with one of the listed AHIMA credentials: Registered Health Information Administrator, Registered Health Information Technician, Certified Coding Associate, or Certified Coding Specialist; or AAPC credentials: Certified Professional Coder, Certified Outpatient Coder, Certified Inpatient Coder, or Certified Risk Coder.
  • A high school diploma or associate degree.
  • Two years of experience in medical coding and billing training, risk adjustment coding, or Hierarchical Condition Category coding, or an equivalent combination of education and experience.
  • Knowledge of medical terminology, anatomy, and physiology.
  • Registered Health Information Technician, Certified Coding Specialist–Professional, or Certified Coding Specialist certification.
  • Experience with various risk adjustment methodologies and chart audits.
  • Strong knowledge of Current Procedural Terminology procedure codes, diagnostic International Classification of Diseases, Tenth Revision, Clinical Modification codes, and/or Medicare Severity Diagnosis-Related Group classification codes, including application of modifiers.
  • Ability to use computer software, with basic math and reasoning skills.
  • U.S. citizenship.
  • Proficiency with Microsoft Office products, including Word, Excel, PowerPoint, Publisher, and related tools and technology.
  • Excellent oral and written communication skills, attention to detail, analytical and evaluation skills, discretion, organizational skills, compliance, and the ability to analyze patients' health records.
Responsibilities
  • Review outpatient health records and electronic medical records for appropriate evaluation and management services.
  • Validate provider, vendor, and internal diagnosis coding by reviewing and analyzing coding samples and claims extracts against actual medical records.
  • Apply critical analysis and coding skills to review outpatient records for quality and comprehensive coding.
  • Prepare and maintain statistical reports and records while ensuring confidentiality of financial and medical records.
  • Resolve questions, inconsistencies, and missing information.
  • Conduct high-level bill reviews.
  • Resolve daily coding scrubber errors to ensure accurate coding structures for correct billing.
  • Resolve claim denials related to coding errors, perform coding charge corrections, and rebill as required.
  • Advise and instruct providers regarding billing and documentation policies, procedures, and regulations.
  • Interact with provider staff regarding conflicting, ambiguous, or nonspecific medical documentation; obtain clarification; and educate providers regarding changes and compliance measurement.
  • Develop preventive measures in response to patterns identified through claims-denial data and Hierarchical Condition Category reviews.
  • Prepare periodic reports on Hierarchical Condition Category findings and trends, identifying corrective measures for clinical staff to resolve denial problems.
  • Compile and report Hierarchical Condition Category statistical data.
  • Ensure work products are thorough, accurate, appropriately documented, and compliant with established criteria and delivery schedules.
Desired Qualifications
  • Certified coder through the American Health Information Management Association or the American Academy of Professional Coders.
  • Knowledge of medical terminology, anatomy, physiology, and pathophysiology.
  • Clinical background and previous chart abstraction experience.
  • Advanced proficiency with Excel, Access, Outlook, PowerPoint, and Word; proficiency with Adobe Acrobat, data analytics tools, and Visio.
