Requirements
  • A minimum of 3 years of experience working with children, adolescents, or families is required.
  • Knowledge of local and regional resources available to the patient population is required.
  • A master's degree in Clinical Social Work, Clinical Counseling, Clinical Psychology, or Marriage and Family Therapy is required.
  • Licensure with the California Board of Behavioral Sciences as a Licensed Clinical Social Worker or Licensed Marriage and Family Therapist is required.
  • American Heart Association Healthcare Provider Basic Life Support certification is required.
  • Live Scan fingerprint clearance required by the Department of Healthcare Services is required.
Responsibilities
  • Support children and adolescent patients and their families who present with complex psychiatric and social problems.
  • Support patient access across all levels of care at Ohana, including referral coordination, intake operations, intake assessments, and appropriate follow-up.
  • Ensure each patient and family experiences an efficient admission process from first contact through entry into care.
  • Triage patients to the appropriate level of care and conduct closed-loop communication across departments and with referral sources.
  • Maintain relationships with referral partners, hospital departments, and community providers to coordinate care and access.
  • Serve as a liaison between Ohana and external referral networks to ensure consistent communication and follow-up.
  • Support psychiatric patients through crisis services, the Safe Harbor urgent care program, adolescent residential care, partial hospitalization, and intensive outpatient programs.
  • Attend treatment team meetings and clinical rounds.
  • Collaborate with therapists, psychiatrists, and other behavioral health staff.
  • Complete other tasks assigned by the leadership team to support patient care at Ohana.
  • Complete organization-provided de-escalation training.
  • Complete mandatory Monterey County 5150 training to obtain 5150 certification.
