Requirements
- A high school diploma or General Educational Development credential is required.
- A Certified Nursing Assistant credential is preferred.
- National Alliance for Direct Support Professionals certification must be obtained within the probationary period and maintained annually.
- AMAP, SCIP, cardiopulmonary resuscitation, and First Aid certifications are preferred.
- A valid driver's license from New York, Pennsylvania, Connecticut, or New Jersey with six or fewer points is required.
- Effective verbal, written, and technological communication skills are required to interact with persons supported, families or advocates, and other professionals.
- The ability to assess situations, identify potential problems, and develop appropriate solutions is required.
- Comprehensive understanding of the needs, challenges, and best practices for supporting persons with developmental disabilities is required.
- Experience working with adults who have developmental disabilities is required.
- Familiarity with regulatory requirements and standards governing residential programs for persons with developmental disabilities is required.
- The ability to organize time effectively, prioritize assignments, and maintain authentic, contemporaneous records is required.
- The ability to handle confidential information under the Health Insurance Portability and Accountability Act effectively and tactfully is required.
- The ability to work effectively as part of a team to deliver continuity of care and dignity to those served is required.
- A strong commitment to safety is required.
Responsibilities
- Assist with activities of daily living, including personal hygiene, food preparation, housekeeping, laundry, shopping, and mobility support.
- Execute behavioral support and monitoring plans and manage medical needs, including wound care, feeding, respiratory care, colostomy care, gastrostomy tube care, frequent medical visits, barrier-free support, wheelchair support, dementia or Alzheimer's-related daily transfers, and support for individuals aged 65 and older.
- Maintain accurate and timely service records of daily activities, medication administration, behavior data, and incidents.
- Maintain facility compliance with Office for People With Developmental Disabilities, Justice Center, and Council on Accreditation standards.
- Facilitate participation in community activities, social events, the advisory board, and recreational opportunities to promote community inclusion and integration.
- Use authorized plans to de-escalate challenging behaviors, use Relias, and respond to crises.
- Participate in Life Plan Meetings and review habilitation plans and oversight documents to ensure value-based outcomes.
- Provide an atmosphere conducive to positive behavior, observe and report regressive behavior, and work cooperatively with clinical staff.
- Manage complex aspects of supporting individuals with dual diagnoses by attending appropriate workshops, training, and meetings.
- Transport persons supported to institutions, hospitals, recreational activities, and home visits, and facilitate day-to-day implementation of their needs.
