Requirements
  • A bachelor's degree in social work or a related field is required, along with one year of experience working with children aged 0–36 months.
  • A master's degree in social work, early childhood education, or a related human services field is strongly preferred.
  • At least two years of post-doctoral work with the developmentally disabled population is required.
  • Applicants must have interviewing techniques that support person-centered thinking.
  • Applicants must understand principles, objectives, and techniques of counseling individuals.
  • Applicants must understand normal and abnormal individual and group behavior and human growth and development of family relationships.
  • Applicants must maintain case records in a clear and concise manner.
  • Applicants must develop standard business communications using proper grammar, spelling, and punctuation.
  • Applicants must convey complex information and ideas clearly, concisely, and professionally in oral communication.
  • Applicants must be able to operate basic office equipment.
  • Applicants must have basic familiarity with Microsoft Word, Microsoft Excel, Microsoft PowerPoint, Microsoft Outlook, or similar software applications.
  • Applicants must be able to facilitate discussions and collaboration, obtain cooperation from applicants and families, empower applicants to develop supports and resources, solve problems creatively, manage crises, make decisions, adapt, and organize work effectively.
  • Applicants must maintain confidentiality in compliance with the federal Health Insurance Portability and Accountability Act and the California Lanterman Act.
Responsibilities
  • Lead the assessment of applicants aged 0–36 months applying for Early Start services.
  • Coordinate interdisciplinary team eligibility assessments and develop the Initial Individualized Family Service Plan within 45 days of referral for eligible applicants.
  • Contact clients, families, or guardians and schedule appointments.
  • Interview applicants and families to obtain social, emotional, developmental, and familial information for home-based assessments and social diagnoses.
  • Obtain intake and application information and conduct needs assessments that prioritize available resources.
  • Clarify service requests and client or family needs.
  • Review and analyze available data and identify barriers and opportunities in clients’ developmental objectives and plans.
  • Design, monitor, and revise Individualized Family Service Plans to meet personal goals and determine client availability within state-mandated timeframes.
  • Assess client and family needs, preferences, and life choices.
  • Confirm immediate needs and provide or arrange necessary interventions or services.
  • Review submitted materials with applicants, families, or guardians within designated timeframes.
  • Provide information about Golden Gate Regional Center, Early Start, and available services.
  • Complete Early Start assessment reports and Applicant Developmental Evaluation Reports.
  • Schedule and coordinate meetings with assessment teams and applicants.
  • Obtain signed releases of information and submit completed application packets and releases to the intake office assistant.
  • Coordinate the assessment team’s completion of the Applicant Developmental Evaluation Report during eligibility determinations.
  • Provide information and referrals to applicants who are not eligible for agency services.
  • Research community resources and services.
  • Collect information for use by the interdisciplinary team and provide intake eligibility services as needed.
  • Provide crisis management and outreach services as assigned.
  • Review and ensure that applicants receive appropriate services from existing community resources.
  • Empower applicants and families to participate fully in coordinating and using available resources.
  • Advocate for applicants in unique circumstances.
  • Maintain applicant records and Case and Title XIX progress notes.
  • Prepare written and verbal reports and complete administrative paperwork.
  • Participate in staff meetings, internal meetings, task forces, community meetings, activities, and special projects.
  • Assist community resources in improving service quality for people with disabilities and identify community service gaps to stimulate service development.
Desired Qualifications
  • Work experience in early intervention, early childhood education, or a regional center.
  • Case management and assessment experience.
  • Experience in multicultural settings or multilingual capability.
