Requirements
  • At least 3 years of direct patient-facing work experience.
  • An active multi-state or compact Licensed Practical Nurse or Licensed Vocational Nurse license is required.
  • Technical aptitude with Microsoft Office Suite is required.
  • Must have active high-speed Wi-Fi.
  • Must have a home office or HIPAA-compliant workspace.
  • The position requires sedentary work with prolonged sitting.
  • The position requires clear speech and attentive listening by telephone for extended periods.

Responsibilities
  • Manage the patient census and close gaps in clinical and non-clinical patient care.
  • Identify and coordinate community resources for patients who would benefit from them.
  • Provide patient education and health literacy support for chronic-condition management.
  • Perform medication management, including identifying medication concerns, medication reconciliation, supporting adherence, and coordinating refills.
  • Help ensure timely delivery of Home Health, durable medical equipment, Home Infusion, and other critical services.
  • Resolve patient questions and maintain open dialogue to understand patient needs.
  • Assist with and manage referrals and appointment scheduling.

  • Additional single-state Licensed Practical Nurse licenses.
