Requirements
  • Possession of a valid multistate Registered Nurse or Licensed Practical Nurse license.
  • Two or more years of Registered Nurse or Licensed Practical Nurse clinical experience.
  • Basic Life Support through the American Heart Association must be obtained within 90 days of hire.
  • Ability to become proficient with the organization’s electronic medical record system.
  • Ability to communicate effectively verbally and in writing and produce logical, clear, and concise documentation.
  • Ability to organize work, prioritize tasks, manage time, multitask, and maintain work integrity despite interruptions.
  • Familiarity or experience communicating with patients, especially by telephone.
  • Ability to make independent nursing decisions and demonstrate knowledge of appropriate patient care.
  • Ability to handle emergency situations calmly and effectively.
  • Ability to maintain confidentiality in accordance with organizational policy and Health Insurance Portability and Accountability Act requirements.
  • Ability to travel throughout the service area when required.
Responsibilities
  • Engage assigned patients and decrease the impact of exacerbations through targeted care management and interdisciplinary care coordination.
  • Implement ongoing care management services to promote continuity of care.
  • Create and periodically update patient-specific care plans with clear goals, priorities, and realistic actions.
  • Perform medication reconciliations, obtain complete medical histories, and conduct health risk assessments and preventive and behavioral health screenings.
  • Schedule annual exams and follow-up appointments, arrange transportation and prescription coordination, and coordinate other preventive care services as needed.
  • Use motivational interviewing and shared decision-making to engage patients in health and social support goals and coach them in managing health conditions and self-care.
  • Monitor and review discharge notifications and information in electronic health record reports and payer portals.
  • Collaborate with outside facilities to obtain timely discharge records and needed information.
  • Contact patients recently discharged from inpatient or emergency department settings to evaluate status, complete medication reconciliation, arrange follow-up care, and support transitions back to primary care.
  • Assess social determinants of health, evaluate patients for chronic care management services, connect patients with community and organizational resources, and discuss findings with providers as appropriate.
  • Communicate necessary information to patients, providers, care teams, and staff through clear and concise documentation.
  • Provide chronic disease education about patients’ conditions, medications, and treatment plans.
  • Use accountable care organization data and informatics, technology, and telehealth platforms to connect patients to care and provide nursing assessments.
  • Contribute to and use data to achieve program goals and improve patient health outcomes.
  • Maintain health promotion, patient education, and community resource materials.
  • Educate providers and support staff about value-based care initiatives related to hospital visits, discharges, and chronic care management.
  • Participate in quality improvement activities related to emergency department utilization and hospital readmissions.
  • Participate in in-service programs, staff meetings, continuous quality improvement, and performance and protocol evaluations and development.
  • Perform duties in compliance with organizational policies and procedures.
