Requirements
  • A bachelor's degree in human services, social work, or a closely related field, or related experience.
  • At least one year of direct, full-time case management experience working with youth experiencing homelessness.
  • Experience working with the literally homeless population, youth, and youth-headed households.
  • Familiarity with current housing market information, housing trends, and available resources.
  • Crisis intervention skills.
  • A Colorado driver's license and state-mandated automobile insurance.
  • Knowledge and skill in applying Critical Time Intervention and Motivational Interviewing.
  • Proficiency in written, oral, and interpersonal communication.
  • Competency with Microsoft Office and cloud-based applications.
  • Ability to work independently and in a team-oriented, collaborative environment.
  • Strong time-management, prioritization, and organizational skills.
  • Ability to use Homeless Management Information System, ServicePoint, and other databases.
  • Ability to provide equitable and inclusive services and ensure participants have an active voice in programming.
Responsibilities
  • Provide service provision and housing assistance to participants in programs housed at the Bannock Youth and Family Center.
  • Conduct needs-based assessments, screenings, community outreach, and intensive case management.
  • Collect documents required to obtain and maintain housing, identification, income, and household information.
  • Attend intakes, housing orientations, lease-ups, and participant educational opportunities.
  • Maintain timely reports in database systems and hard-copy files according to grant guidelines.
  • Use the OneHome system as required by funding sources.
  • Assist participants with housing searches, rental applications, lease interpretation, and understanding tenant rights and responsibilities.
  • Provide housing navigation and process referrals for participating households.
  • Coordinate services with the team, service providers, and multidisciplinary teams.
  • Assist with relocating youth and securing emergency housing.
  • Document client interactions, eligibility information, demographic information, and literal homelessness documentation.
  • Facilitate referral, intake, and orientation processes for new YTP households.
  • Ensure participant housing complies with program standards.
  • Complete housing quality inspections, rent reasonableness assessments, and fair market rent assessments.
  • Attend orientations, trainings, educational programs, staff meetings, community meetings, conferences, and workshops as requested.
  • Maintain confidentiality and perform duties in accordance with the Social Work Code of Ethics.
  • Use a personal vehicle for service provision and housing-related activities across a seven-county area.
  • Work full time, Monday through Friday, with some evenings and weekends required.
