Merck's oncology wins lift outlook, but 110x P/E clouds its $141.82 share price

Merck (MRK) is drawing investor attention after oncology trials showed meaningful survival benefits for KEYTRUDA, sacituzumab tirumotecan, and a melanoma vaccine developed with Moderna. The shares closed at $141.82, down 4.37% over seven days and 4.68% over 30 days. They are still up 33.23% this year and 67.74% over one year in total shareholder return. A popular narrative puts fair value at $159.76, making the stock 11% undervalued. Merck plans more than 20 new growth drivers, with management pointing to over $70 billion in mid-2030s revenue potential to offset the KEYTRUDA loss of exclusivity. Valuation remains contested. Merck trades at 110.3 times earnings, against a fair ratio of 50.2, a US pharmaceuticals average of 15.7, and peers at 33.9. Its profit margin is 4.8%.

ESMO 2026: Moderna vaccine debate, Roche's $3.6B KRAS bet and Amgen's lung cancer data

Three cancer drug trials drew attention at the European Society for Medical Oncology (ESMO) meeting, with investors and analysts weighing the results. Moderna and Merck's vaccine intismeran autogene cut the relative risk of melanoma recurrence or death by 42% in a Phase 3 trial. Moderna's shares have still fallen by double digits since the results leaked early, as investors question its valuation and the vaccine's manufacturing and commercial potential. Roche's divarasib reduced the risk of death or disease progression by 42% in KRAS G12C lung cancer, beating Krazati and Lumakras. Patients lived a median of about 18 months, versus a year. Roche raised its peak sales forecast from 3.6 billion Swiss francs to 3 billion. Amgen's Imdelltra, combined with Imfinzi, cut the risk of death by 58% in small cell lung cancer. Median survival was 21.1 months, against 12.7 months for Imfinzi alone.

Vaccine reduces risk of melanoma recurrence, death by 42%: What to know.

Vaccine reduces risk of melanoma recurrence, death by 42%: What to know. By: Elizabeth Gregerson Receiving a personalized mRNA-based cancer vaccine alongside immunotherapy lowered the risk of recurrence or death by 42% versus immunotherapy alone in a phase 3 clinical trial of patients with surgically resected stage 2B to 4 melanoma. The vaccine, called intismeran autogene and jointly developed by Merck and Moderna, was tested in combination with Merck's immunotherapy Keytruda after surgery as part of the phase 3 INTerpath-001 trial, according to an Oct. 22 news release from the companies. The trial enrolled 1,137 patients, about two-thirds of whom were randomly assigned to receive the vaccine. The vaccine is given in nine doses and is personalized to include up to 34 neoantigens, or proteins specific to each patient's tumor, according to The New York Times. The trial results were leaked online Oct. 22, The Wall Street Journal reported, two days before Merck and Moderna were expected to present the data at the European Society for Medical Oncology conference in Madrid. The leak prompted Merck, Moderna and ESMO to release the full data early. Here are six notes on the trial results: * In addition to lowering the risk of recurrence or death by 42%, intismeran plus Keytruda lowered the risk of distant metastasis or death by 38% compared with Keytruda alone. * Outcomes in the vaccine group were similar to those for Keytruda alone during the first six months while patients completed the vaccine regimen. After six months, a sensitivity analysis showed a 58% lower risk of recurrence or death and a 50% lower risk of distant metastasis or death versus Keytruda alone. * Results were generally consistent across patient subgroups, and no new safety signals emerged. * Robert Vonderheide, MD, president-elect of the American Association for Cancer Research, told the Times the results are on par with "our biggest breakthroughs." However, neither this trial nor an earlier phase 2 trial has yet shown whether the vaccine improves overall survival, the Times reported. Overall survival is considered the gold standard for determining whether a cancer treatment works and can be a requirement for regulatory approval. * Merck plans to apply for regulatory approval as quickly as possible, Marjorie Green, MD, head of oncology clinical development, told the Times. * Personalized cancer vaccines have also shown promise in small studies in pancreatic, kidney and brain cancers, but not all trials have succeeded. BioNTech ended a phase 2 colorectal cancer trial of its personalized vaccine in August after a recommendation from an independent data safety monitoring board. Moderna sinks 18% after leak of melanoma vaccine data showing 42% lower recurrence risk

Moderna shares fell 18% after detailed Phase 3 results for its personalised melanoma vaccine, developed with Merck, appeared online two days before their scheduled presentation. The data surfaced on medical news site Oncology Central and was taken down shortly afterwards. Trading in Moderna was halted for volatility. The leaked data still looks positive. Adding the vaccine, intismeran autogene, to Merck's Keytruda cut the risk of recurrence or death by 42% versus Keytruda alone. After two years, 82.2% of patients on the combination were alive and cancer-free, against 70% on Keytruda alone. The trial enrolled 1,137 patients. The stock had risen more than eightfold this year, and traded at about 52x forward sales before the drop. The 42% figure also trails the 49% from the smaller Phase 2b trial. Full results are due at the ESMO Congress on Saturday.
