Requirements
  • Must have a valid license in the state or states of practice, or proof of license eligibility.
  • Must have a valid driver's license or reliable, independent transportation to and from patients' residences.
Responsibilities
  • Provide outpatient rehabilitation services in patients' homes or in rooms at independent living facilities for older adults.
  • Provide physical therapy treatments under the supervision of a Licensed Physical Therapist to facilitate increased patient independence and functioning.
  • Attend and contribute to patient care, staffing conferences, and other required meetings as a representative of physical therapy under the direction of the Physical Therapist.
  • Adhere to department and patient treatment schedules, timely clinical and billing documentation requirements, and work-related meetings.
  • Work with rehabilitation team members to implement programs and activities consistent with each patient's needs and capabilities.
  • Assist the Physical Therapist with tests, evaluations, and complex procedures.
  • Instruct patients, families, and caregivers in the skills and techniques of the physical therapy treatment program under the Physical Therapist's supervision.
  • Maintain appropriate and timely documentation for all patients treated.
  • Clean and maintain equipment, and identify and report repair needs.
  • Ensure patients and families receive high-quality service in a caring and compassionate atmosphere that recognizes their needs and rights.
  • Implement each patient's individualized treatment plan established by the primary Physical Therapist.
  • Provide individualized physical therapy treatments, including therapeutic exercise, functional training, manual therapy, airway clearance techniques, integumentary repair and protection, electrotherapeutic and other physical agent or mechanical modalities, health and wellness interventions, falls-risk interventions, and chronic disease management education.
  • Perform other related duties as required.
