  • Must complete annual educational requirements and maintain regulatory requirements.
  • Must maintain patient confidentiality and comply with organizational policies, applicable laws, regulations, and the Code of Conduct.

  • Must occasionally lift materials up to 20 pounds and have near-normal vision and hearing.
  • Must manage demanding schedules, including evening and weekend commitments, and be available outside standard business hours for community program needs, grant deadlines, partnership engagements, and executive responsibilities.
Responsibilities
  • Establish and maintain an enterprise-wide framework for quality measurement and continuous improvement across community programs and service lines, maintaining dotted-line alignment with hospital and nursing home quality teams rather than directing their operations.
  • Lead HAMC's population health strategy, including oversight of the AHEAD Model implementation and population health initiatives at Miller County Medical Center as the HUB.
  • Partner with clinical, operational, and community teams to design and execute programs addressing social determinants of health, reducing preventable hospitalizations, and improving community health outcomes.

  • Lead population health partnerships with state agencies, federally qualified health centers, and regional health networks.
  • Use quality outcome data to evaluate program effectiveness, identify community health access gaps, and drive interventions to reduce preventable hospitalizations and emergency department use.
  • Oversee development, launch, and ongoing performance of community-based service programs, defining operational standards, staffing models, quality benchmarks, and financial sustainability targets.

  • Provide executive leadership and accountability for program administrators and service line leaders within population health.
  • Develop and maintain quality dashboards, scorecards, and performance metrics; interpret clinical and operational data to identify trends, risks, and improvement opportunities.
  • Present enterprise quality findings, including dotted-line program data from hospital and nursing home quality teams, to the CEO, Board, and executive leadership team.
  • Ensure leaders and staff in directly managed programs have training, tools, and quality improvement skills; partner with Human Resources and Staff Development to integrate population health competencies into professional development.
  • Develop and maintain program quality standards, outcome benchmarks, and performance expectations, including regular performance reviews and documented improvement plans where needed.
  • Engage patients, families, staff, and community partners in quality improvement initiatives and communicate outcomes and improvement goals throughout the organization.
  • Partner with the Chief Population Health Officer and executive leadership to integrate population health quality objectives and community program outcomes into strategic and annual operating plans.
  • Identify and pursue opportunities to expand population health programs and community service lines through new programs, partnerships, and grant-funded initiatives; assess feasibility, define program models, and advance proposals through approval processes.
  • Provide strategic direction and clinical/programmatic expertise to support business development without direct management authority over business development staff.
  • Partner with marketing and communications leadership on messaging, campaigns, and community engagement strategies; represent HAMC at community events, health fairs, and public forums.
  • Attend occasional off-site regulatory meetings, quality conferences, and collaborative network engagements; travel to community events and regulatory visits as needed.
Desired Qualifications
  • A doctoral degree, such as a Doctor of Nursing Practice, PhD, or MD.
  • Certification in population health management (CPHM) or equivalent; Certified Professional in Healthcare Quality (CPHQ) is accepted with demonstrated population health experience.
  • Lean Six Sigma Green Belt or Black Belt certification.
  • Experience with population health models, value-based care programs, and CMS rural health initiatives, including the AHEAD Model or similar.
