- Must be a nurse in good standing and currently licensed by the state.
- Previous experience in a hospital, long-term care facility, or other healthcare-related facility is required.
- Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as the laws, regulations, and guidelines governing long-term care.
- Must have computer skills, including Windows applications.
- Prior experience completing Minimum Data Sets and care plans is required.
- Demonstrated proficiency in Resident Assessment Instrument/Prospective Payment System processes is required.
- Must maintain a current Basic Life Support/Cardiopulmonary Resuscitation certificate, excluding assisted living facilities and independent living facilities.
- Participate in the Resident Assessment Process by completing pertinent items on the Minimum Data Set and accompanying Care Area Assessments to ensure appropriate patient care and nursing center reimbursement.
- Direct the nursing center interdisciplinary Resident Assessment Instrument process while following guidelines for completion, computer encoding, and electronic state transmission of the Minimum Data Set.
- Participate in developing individualized resident care plans, identifying resident needs and projected outcomes as required by federal and state regulations.
- Train new interdisciplinary team members on clinical content and electronic completion of applicable Minimum Data Set items.
- AccuMed experience is preferred.
- Previous experience as a Minimum Data Set Coordinator is preferred.
