Requirements
  • Be registered and in good standing with the College of Nurses of Ontario.
  • Hold an RPN diploma or an RN diploma or degree from a recognized university or college.
  • Hold valid First Aid or CPR certification.
  • Have demonstrated expertise and nursing experience providing palliative and end-of-life care; candidates without CAPCE certification must demonstrate significant palliative care experience and commit to obtaining certification.
  • Demonstrate effective written and verbal communication and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • Commit to lifelong learning and professional growth, and apply critical thinking skills to provide client care.
  • Hold a valid Ontario G2 or G driver’s license and have a reliable vehicle for travel.
  • Be available for assigned morning or evening shift hours and participate in a standby/on-call rotation.
Responsibilities
  • Assess and monitor clients, and develop care plans and coordinate care.
  • Evaluate care plan interventions through ongoing reassessment and evaluation results.
  • Ensure quality patient care by identifying and acting on factors that affect practice autonomy and the delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability when interacting with clients and families.
  • Support safe client care by developing knowledge and skills in accordance with continuous quality improvement principles.
  • Inform the nursing supervisor of client activities, relevant concerns, or changes in a client’s condition or care plan, and submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.
  • Participate in the standby/on-call rotation, answer calls, and respond when needed.
  • Participate in team meetings as required during stated availability.
  • Provide and coordinate home health nursing services to individuals, families, and groups in accordance with College of Nurses of Ontario practice standards, VHA policies and procedures, program requirements, and community health nursing standards.
  • Collaborate with external palliative care community teams, care coordinators, physicians, nurse practitioners, and personal support workers.
Desired Qualifications
  • Membership in RNAO or WeRPN is preferred.
  • Comprehensive Advanced Palliative Care Education (CAPCE) in Hospice Palliative Care Nursing is strongly preferred; candidates without it must commit to obtaining the certification after joining VHA.
  • Wound care or venipuncture knowledge and experience is an asset.
  • Acute care and medical-surgical nursing experience is an asset.
  • Knowledge of community nursing is an asset.
