Requirements
  • Five or more years of nursing experience, preferably in an acute or long-term care setting.
  • Two or more years of supervisory experience.
  • A degree from an accredited school of nursing.
  • A current New York State nursing license in good standing.
  • Proven registered nurse patient assessment skills.
  • Current knowledge of best-practice nursing standards and care.
  • Proficiency in the care planning process.
  • Ability and knowledge to respond in emergency situations.
  • Ability to analyze problems, plan resolution strategies, implement solutions, and evaluate outcomes.
  • Fluency in English, including reading, writing, and speaking.

  • An equivalent combination of education and experience may be substituted when it provides proficiency in the listed areas of responsibility.
Responsibilities
  • Provide clinical oversight for high-acuity patients, including post-surgical, orthopedic, cardiac, neurological, and medically complex cases.
  • Guide and support the team through high-acuity admissions.
  • Lead interdisciplinary collaboration with therapy, physicians, case management, and pharmacy to drive outcomes and discharge planning.
  • Ensure timely admissions, assessments, and care plan development to meet regulatory and clinical standards.
  • Monitor and improve key quality metrics, including rehospitalization rates, infection control, falls, length of stay, and functional outcomes.
  • Supervise, mentor, and develop nursing staff in a fast-paced, high-turnover environment.
  • Ensure accurate and timely documentation, including skilled documentation supporting reimbursement under the Patient-Driven Payment Model.
  • Lead performance-improvement initiatives and regulatory readiness.
  • Manage the clinical care-planning process to coordinate and deliver quality patient care, and review and update care plans.
  • Provide or assist with direct care when needed, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.
  • Ensure a safe and secure environment for clients, residents, staff, and visitors.
  • Assess residents upon admission and periodically as conditions change, and conduct assessments as needed.
  • Obtain and implement physician orders, checking for accuracy and completeness.
  • Communicate effectively with clients, residents, families, and interdisciplinary teams, and ensure effective staff communication.
  • Maintain customer service within the area of responsibility.
  • Provide leadership and direction to personnel in accordance with organizational goals and mission.
  • Enforce the bargaining-unit contract fairly and consistently, addressing issues in a timely and appropriate manner.
  • Plan, implement, and evaluate staff clinical competencies and ongoing educational needs; update orientation and training materials as needed.
  • Share current knowledge and clinical expertise through formal and informal education, and maintain current clinical knowledge.
  • Hold required monthly staff meetings across all shifts.
  • Mentor new employees and foster collaborative working relationships.
  • Ensure patient care is delivered according to best-practice standards.
  • Develop, review, and revise policies, procedures, and applicable documentation to meet regulatory agency standards.
  • Interpret policies, procedures, and regulations for staff.
  • Ensure clinical staff understand and comply with policies, procedures, and regulations.
  • Monitor adherence to patient-care standards through quality-assurance and performance-improvement processes.
  • Participate in the development, implementation, and evaluation of quality-assurance and performance-improvement activities.
  • Serve as an active member of quality-assurance and performance-improvement committees.
  • Direct data collection for complaint investigations, incident reports, and critical-incident investigations in collaboration with the quality-assurance department.
  • Summarize investigation information and act after consultation with the Director and quality-assurance committee.
  • Review the Point of Right process to make improvements and changes in quality measures.
  • Keep the Director informed of patient-care problems and issues in a timely manner.
  • Manage unit expenses and costs, including staff overtime, linens, and supplies.
Desired Qualifications
  • A Bachelor of Science in Nursing or Bachelor of Science in a related field is preferred.
  • Nursing experience in an acute or long-term care setting is preferred.
