Requirements
  • A High School Diploma or GED is preferred, although relevant background experience may be considered in lieu of education.
  • CPR and First Aid certification is required, with training provided.
  • AMAP certification is required, with training provided.
  • SCIP certification is required, with training provided.
  • Program-specific training is required, with training provided.
  • A valid New York State driver’s license acceptable to agency insurance standards is required depending on program needs.
  • Maintain valid vehicle insurance and provide an insurance card when using a personal vehicle.
  • Maintain a professional, kind, courteous, patient, and tolerant manner.
  • Acquire and maintain all required certifications in accordance with agency policy.
  • Attend required trainings, meetings, updates, and medical requirements.
Responsibilities
  • Implement and document each person’s total plan of care while encouraging maximum independence and self-worth.
  • Provide, encourage, assist, and supervise people served with personal care and activities of daily living while respecting their privacy and dignity.
  • Assist with bathing, toileting, dressing, feeding, transportation, meal preparation, inventory, laundry, housekeeping, infection control, and purchase of supplies.
  • Perform medical and health-related functions in accordance with AMAP policy, including medication administration and treatments after satisfactory completion of the AMAP course.
  • Implement prescribed therapies or treatments ordered by licensed physicians, nursing staff, or therapists.
  • Transport or escort people to program, medical, dental, and other appointments as needed.
  • Accompany people and assist them in participating in community activities.
  • Comply with state, federal, agency, and OPWDD policies, procedures, and regulations.
  • Maintain accurate, compliant, and up-to-date records for people served.
  • Operate agency vehicles in accordance with agency policies, procedures, and traffic laws, and document vehicle maintenance and condition.
  • Manage individual funds and petty cash and ensure the appropriate use of individual and agency property and equipment.
  • Assist with the coordination, participation, communication, and documentation of meetings for people served.
  • Maintain a calm, tolerant, kind, courteous, and accepting environment in accordance with each person’s service plan and plan of protective oversight.
  • Provide and ensure a safe work and living environment in compliance with safety policies, procedures, and regulations.
  • Attend and participate regularly in staff meetings and mandatory meetings.
  • Perform behavior management using SCIP techniques when necessary and in compliance with policy and behavior plans.
  • Foster community inclusion by involving individuals in planning and implementing recreational and leisure activities.
  • Respect individuals’ rights and responsibilities and immediately report concerns involving potential harm, mistreatment, or rights restrictions.
  • Work at agency residences throughout the organization when requested to ensure appropriate coverage.
  • Participate as an active member of each individual’s program team.
  • Maintain schedule flexibility to ensure individual care and program implementation.
  • Perform other related duties as requested by the supervisor.
