Physical Therapist Job Responsibilities

As a Home Health Physical Therapist, you will provide rehabilitation services that help patients recover from illness, injury, surgery, or chronic conditions while remaining safely in their homes.

Key Responsibilities

Evaluate and treat patients in their homes using evidence-based physical therapy interventions
Develop and implement individualized treatment plans that promote mobility, strength, balance, and functional independence
Assess patient progress and adjust treatment plans as appropriate
Educate patients and caregivers on home exercise programs, fall prevention strategies, mobility techniques, and safety recommendations
Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals

• Rehab Therapist Patient Documentation (Novice): Demonstrates proficiency in documentation practices. Documents in the appropriate templates and utilizes only approved abbreviations within patient documentation. Meets institutional standards for quality and timeliness of documentation.
• Treatment Planning & Goal Setting (Novice): Demonstrates sufficient proficiency in developing a plan of care that incorporates treatment interventions focused on meeting objective goals. Develops and documents patient focused short and long term goals that are functional and measurable. Identifies the need for and referral to other services, as needed.
• Rehabilitation Therapist Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining information pertinent to the assessment and administering and scoring standardized tests and objective measures of deficit areas. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.
• Treatment Implementation (Novice): Demonstrates proficiency in providing treatment consistent with physician prescription and established plan of care. Able to modify treatment plan as needed to demonstrate provision of skilled therapy services including progression of treatment throughout course of care, and revision of treatment interventions of goals not met. May supervise and direct certified athletic trainers (PT only), physical therapist / occupational therapist assistants, technicians and volunteers and students. Selects and follows correct protocol for care and utilize evidence based treatments. Has completed at least one year of clinical experience in treatment implementation.
• Physical/Occupational Therapist Patient Education (Novice): Demonstrates proficiency in communicating functional impact of assessment findings, treatment interventions, treatment planning diagnosis, and disease process. Considers the communication needs/level of understanding of audience and may share appropriate information/literature. May supervise, educate and train students from professional programs of physical therapy, occupational therapy and athletic training.

Jasmiry (Jas) Bennett, DNP, MBA, APRN, ACNP-BC, has been named the inaugural Vice President and Chief Advanced Practice Officer for Vanderbilt Health, according to an announcement Aug. 12 from System Chief Nursing Officer Karen Keady, PhD, RN, NEA-BC. Bennett, currently Advanced Practice Director at Yale New Haven Hospital, will begin her new role in November. "Dr. Bennett is a highly respected health care leader with more than 20 years of progressive experience in clinical practice, operations and executive leadership," Keady said. Vanderbilt Health, supported by a grant from Eli Lilly, is developing an AI-powered tool within their EHR system to expedite Alzheimer's treatment, focusing on reducing delays in monoclonal antibody therapy. Aug. 6, 2026 Vanderbilt Health has put together a project to accelerate the process of getting patients to Alzheimer's treatment, thanks to a grant from Eli Lilly and Company. The project is detailed in a news article by Paul Govern. Through the 18-month, $600,000 initiative, the team will develop an artificial intelligence (AI) tool inside their electronic health record system (EHR). The device will help reduce the time that patients wait to receive monoclonal antibody therapy after their healthcare provider refers them. Amalia Peterson, MD, Assistant Professor of Neurology and a co-principal investigator on the project told Govern that the AI agent will not replace clinical judgement. Instead, "it will remove a lot of the friction that keeps patients waiting." The agent will work by summarizing a patient's relevant chart information once they are recommended by a clinician for cognitive concern, and reporting gaps that might slow evaluation. It will also "recommend whether a case should be routed as priority or standard," however, "clinicians will retain the ability to accept, edit or override every recommendation." The team is considering developing additional AI agents within the EHR to streamline other areas of Alzheimer's care, but will first focus on helping patients receive their first monoclonal antibody infusion sooner. Vanderbilt Health, Siemens Healthineers sign $87 million technology partnership. DOTmed July 28, 2026 Gus Iversen Vanderbilt Health has entered a multiyear, $87 million Value Partnership with Siemens Healthineers to upgrade diagnostic imaging and radiation oncology technology across its Nashville-area hospitals. The agreement covers equipment modernization, technology standardization, service operations and workforce development programs. According to the organizations, the partnership also establishes a framework for technology planning, installation support and operational performance initiatives aligned with Vanderbilt Health's clinical and operational priorities. The Nashville, Tennessee-based health system said the agreement is designed to streamline future purchases and upgrades of imaging and radiation oncology equipment while supporting broader operational initiatives.
