Requirements
- An active Texas physical therapist assistant license in good standing is required.
- Strong clinical judgment and the ability to work independently in the field within PTA scope and under PT supervision are required.
- Must be comfortable escalating changes in condition to a physical therapist, registered nurse, or physician and clearly documenting the rationale.
- Strong communication and documentation skills are required.
- Reliable follow-through on treatment plans, visit cadence, and patient goals is required.
- A current driver's license and comfort with daily travel across the assigned territory are required.
- Must be able to carry or transport basic therapy equipment as needed.
- Competency in core home health PTA scope is required, including safety-first transfers, gait training, balance and fall prevention, assistive-device education, strengthening, functional mobility, home safety assessment and modification education, and patient/caregiver coaching with practical home programs.
Responsibilities
- Deliver skilled PTA visits in patients' homes under the supervising physical therapist's plan of care.
- Execute treatment plans focused on functional mobility and measurable progress, including gait and balance, transfers, strengthening, home safety, and fall prevention.
- Set and track objective progress within PTA scope and communicate updates or concerns to the supervising physical therapist.
- Coordinate with the multidisciplinary care team by escalating changes in condition, aligning on orders, and keeping care coordination moving.
- Coach patients and caregivers on practical home programs.
- Document promptly and thoroughly in Homecare Homebase.
Desired Qualifications
- Prior home health experience is preferred.
- Experience with Homecare Homebase or a similar home health electronic medical record is preferred.
- Deeper clinical experience with orthopedic or neurological populations is preferred.
- Strong organization and time-management skills are preferred.
