Requirements
  • A Master of Social Work degree from a school of social work accredited by the Council on Social Work Education is required.
  • The candidate must be minimally licensed in the state where the Oak Street Health Center is located.
  • Fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities served is required where necessary.
  • Proficiency with Microsoft, GSuite, electronic medical records, and general computer systems is required.
  • US work authorization is required.
  • The candidate should be motivated and experienced, have a problem-solving orientation, and maintain a flexible and positive attitude.
  • Experience working with Medicare-eligible populations, including older adults, adults with disabilities, and adults with severe and persistent mental illness, is relevant to the role.
Responsibilities
  • Provide comprehensive assessments, telephonic check-ins, and in-person visits to the highest-need patients.
  • Partner with patients and their support persons to identify goals, increase healthcare engagement, address barriers, and execute care plans.
  • Maintain knowledge of community resources and make appropriate referrals to meet patients’ healthcare and psychosocial needs.
  • Navigate patient insurance benefits in collaboration with the Patient Relations Manager to support mindful utilization and maximize plan benefits.
  • Provide longitudinal and episodic comprehensive care coordination to help patients independently manage their health conditions and improve quality of life.
  • Educate patients about supportive care options, including advance care planning and end-of-life care, and provide goals-of-care counseling, support, and advocacy throughout the patient journey.
  • Provide psychoeducation to patients and families about life-stage and disease-specific issues, including dementia, adjustment to illness, aging, and caregiver support.
  • Collaborate with care teams to generate solutions to patient issues and lead interventions involving complex psychosocial dynamics.
  • Complete concise and comprehensive documentation on time so care team members can see the status of Medical Social Worker interventions.
  • Partner with Community Health Workers and provide clinical guidance and consultation regarding complex patient needs.
  • Respond to crises and urgent needs by prioritizing work, triaging situations, and multitasking.
Desired Qualifications
  • Prior experience with complex care coordination and/or population healthcare management.
  • Familiarity with using metrics and data to guide practice and drive outcomes.
  • At least one year of medical social work experience in a healthcare environment, preferably within the last two years.
  • Experience working on a multidisciplinary team of healthcare providers.

CVS Omnicare completes $250M bankruptcy sale after $440M government settlement

CVS Health's Omnicare division has completed its Chapter 11 bankruptcy liquidation after selling its business operations for $250 million. A Texas bankruptcy judge approved the wind-down plan following a $440 million settlement with the Justice Department over improper billing practices. Omnicare, which served nursing homes and long-term care facilities, filed for bankruptcy in September 2025 after facing a $949 million judgement for fraudulently dispensing drugs without valid prescriptions and billing federal healthcare programmes for false claims. The settlement requires CVS to pay $130 million upfront and cover the remaining $310 million if Omnicare fails to do so by March 2028. GenieRx Holdings, a joint partnership between Milrose Capital and Integro Asset Management, purchased Omnicare's operations. The sale is expected to close next month.
