Requirements
  • Must possess, as a minimum, a nursing degree from an accredited college or university, or be a graduate of an approved Licensed Practical Nurse or Licensed Vocational Nurse program.
  • Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse in this state.
  • Must be able to read, write, speak, and understand the English language.
  • Must be able to read technical procedures and comprehend policy and procedure manuals.
  • Must be able to apply fractions, percentages, ratios, and proportions to practical situations.
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents served.
  • Must possess knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
  • Must possess leadership and supervisory ability and be willing to work harmoniously with and supervise other personnel.
  • Must maintain required continuing education and licensing and remain in good standing with the State Board of Nursing.
  • Must hold current CPR certification.
  • Knowledge and experience with PCC are preferred, but not required.
  • Must be able to move intermittently throughout the workday and perform the stated lifting, pushing, pulling, and carrying requirements.
  • Must be able to function independently and work effectively with residents, personnel, and support agencies.
  • Must meet the facility's general health requirements, including a medical and physical examination.
  • Must be able to assist with resident evacuation during emergency situations when necessary.
Responsibilities
  • Ensure assigned residents receive consistent care in the skilled nursing facility.
  • Administer medication passes and treatments as ordered.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and facility guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Prepare and submit required reports, recommendations, census reports, charge slips, medical forms, evaluations, studies, and documentation.
  • Admit, transfer, and discharge residents as required.
  • Complete and maintain resident records, charts, nurses' notes, care plans, discharge plans, transfer forms, and accident or incident reports.
  • Receive physician telephone orders and transcribe orders to resident charts, medication cards, treatment plans, and care plans.
  • Prepare and administer medications as ordered and verify resident identity before administration.
  • Maintain medication, narcotic, supply, and equipment inventories and report discrepancies or shortages.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel actions to the Nurse Supervisor.
  • Develop work assignments, provide leadership to assigned nursing personnel, and make rounds to monitor work performance.
  • Ensure nursing assistants and nurse aide trainees meet applicable training and supervision requirements.
  • Provide orientation, in-service training, clinical supervision support, and continuing education for nursing personnel.
  • Coordinate resident admissions, physician rounds, diagnostic and therapeutic services, treatments, rehabilitation, social programs, and discharge transportation.
  • Monitor residents' physical and emotional status, notify physicians and families of changes in condition, and provide required nursing services.
  • Perform catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage-bag care, specimen collection, vital-sign measurement, restorative care, and other professional nursing services as required.
  • Maintain clean, safe, and sanitary nursing areas and enforce hand hygiene, infection control, safety, and personal protective equipment procedures.
  • Review and update resident care plans and ensure direct-care personnel follow them.
  • Participate in quality assurance, facility surveys, infection control, safety, disaster preparedness, and fraud-reporting activities.
  • Protect resident health information, facility information systems, user credentials, and passwords.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. He previously held CFO roles at Capital Senior Living Corporation, managing 128 communities across 23 states, and Belo Corp. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Under his leadership, the company expanded to 323 facilities across 17 states, serving over 31,700 patients daily and generating $5.29 billion in revenue in 2025, representing 29.3% year-on-year growth. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
