- An active Texas physical therapist license in good standing is required.
- Strong clinical judgment and the ability to work independently in the field are required.

- A current driver's license and comfort with daily travel across the assigned territory are required.
- Ability to carry and transport basic therapy equipment as needed is required.
- Competency in core home health physical therapy scope is required, including transfers, gait training, balance and fall prevention, assistive device recommendations, strengthening, functional mobility, home safety assessment and modification education, and patient/caregiver coaching with practical home programs.
- Own the caseload as a case manager by being accountable for each patient's therapy plan, progression, and outcomes.
- Complete comprehensive in-home assessments and deliver skilled physical therapy visits.
- Build and execute plans of care addressing gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set measurable goals, track objective progress, and adjust treatment when patients stall.
- Coordinate with the multidisciplinary care team by communicating changes in condition, aligning on orders, and keeping care activities moving.
- Manage treatment and outcomes directly, including physical therapist assistant supervision where appropriate.
- Coach patients and caregivers on practical home programs.
- Prior home health experience.
- Experience with Homecare Homebase or a similar home health electronic medical record system.
- Deeper clinical experience with high-signal physical therapy populations.
- Experience supervising or mentoring physical therapist assistants and collaborating on care plans.
- Strong organization and time-management skills.
