Requirements
  • Ability to provide peer counseling and support based on common experiences as a peer.
  • Ability to participate in ongoing psychosocial assessments of clients' psychological, social, vocational, systemic, and economic goals and needs.
  • Ability to assess clients' ability to obtain and maintain housing and identify problem behaviors requiring intervention.
  • Ability to develop housing rehabilitation plans that help clients establish skills to find and maintain housing.
  • Ability to monitor, compile, and submit PATH program data, reports, and paperwork.
  • Ability to provide individual supportive case management to help clients identify and cope with symptoms of mental illness affecting their ability to obtain and maintain housing.
  • Knowledge of psychiatric treatment and rehabilitation, public and private community resources, services and benefits, public income maintenance programs, federal and state laws, and agency policies and procedures.
  • Ability to meet the program's billable-hour expectation for direct services.
  • Ability to complete required documentation accurately, completely, and timely according to agency policies and procedures.
  • Ability to maintain the safety, security, and confidentiality of consumers and their families.
Responsibilities
  • Participate in outreach services to prospective clients and coordinate outreach efforts with outside agencies.
  • Serve as a mentor to clients to promote hope and empowerment.
  • Develop ongoing housing rehabilitation plans for clients.
  • Assist clients in obtaining housing.
  • Organize, design, and develop program structure in consultation with the Manager of Housing & Homeless Services.
  • Develop or review manuals pertaining to program policy, procedures, and operating instructions.
  • Develop specific procedures and policies necessary for newly designed or modified programs in consultation with the Manager of Housing & Homeless Services.
  • Provide interventions and referrals to assist consumers in meeting treatment and level-of-care needs, including counseling, case management, vocational rehabilitation, system coordination, consumer advocacy, and crisis management.
  • Ensure treatment planning and interventions are client-centered, culturally competent, and age appropriate, while respecting confidentiality, safety, and self-determination.
  • Use community and natural supports to integrate consumers into their communities and graduate them from services when clinically indicated.
  • Maintain accurate, complete, and timely documentation and participate in agency tracking systems and chart audit procedures.
  • Seek supervisory or collegial assistance when documentation quality or timeliness problems persist.
  • Identify program needs and address them through collective processes.
  • Participate in agency meetings, serve on working committees, and undertake independent projects to improve the agency and enhance services.

  • Housing case management experience is a plus.
