Requirements
  • A bachelor's degree in social work, public health, community health, or a related field is preferred.
  • At least 5 years of experience providing intensive case management to transitional aged youth and/or similarly disenfranchised and high-need populations in social service settings.
  • Demonstrated success providing one-to-one counseling and case management.
  • Knowledge of transitional aged youth needs and resources in the San Francisco Bay Area, especially for people experiencing poverty, homelessness, mental health challenges, substance misuse, trauma, and related experiences.
  • Experience facilitating weekly activities, psychoeducation groups, workshops, or similar activities that support client confidence, wellness, and community connection.
  • Ability to work independently, demonstrate initiative, remain dependable, and maintain a consistent presence.
  • Ability to collaborate with internal and external stakeholders to coordinate care for shared clients and help clients stabilize and reach individual goals.
  • Strong written and oral communication and interpersonal skills.
  • Attention to detail and strong organizational, time-management, and problem-solving skills.
  • Ability to use Windows, Microsoft Office Suite, Salesforce or other database systems, Zoom or other virtual meeting systems, and search engines for research.
Responsibilities
  • Initiate and conduct outreach with transitional aged youth at the center and at offsite locations where they access services or congregate.
  • Administer intake and assessments to enroll clients into the continuum of care, establish baseline safety and health needs, and develop individual goal and service plans with actionable care-coordination steps.
  • Develop care-coordination plans from one-to-one sessions, referral information, and assessments; identify care-coordination needs within integrated services and partner organizations; and establish successful linkages to care.
  • Teach and coach clients to self-advocate within complex service systems using effective communication tools and strategies.
  • Identify knowledge and skills gaps affecting ongoing stability and success, and link clients to educational resources such as sexual health education, personal hygiene, pet care, harm reduction, healthy relationships, adult literacy, digital literacy, and banking resources.
  • Coordinate, facilitate, and/or conduct expressive arts activities, therapeutic activities, or psychoeducation groups supporting client confidence, wellness, community connection, and growth mindset.
  • Monitor client progress through scheduled appointments and follow-up to ensure implementation of individual service plans.
  • Maintain accurate, timely, and up-to-date records of intake, assessments, progress, and outcomes in required databases and hard-copy files.
  • Participate in staff meetings, staff training events, supervisory meetings, and community meetings to maintain relationships with programs, community partners, and city agencies and enhance service delivery and client support.
