  • Current Licensed Vocational Nurse license for the State of California.
  • Current Basic Life Support and cardiopulmonary resuscitation certification.
  • Successful completion of competency sign-off and orientation within 90 days of hire.
  • Ability to communicate via telephone, video, and/or in person.
  • Ability to read documents, computer screens, and forms.
  • Ability to remain stationary for extended periods.
  • Ability to lift, carry, or otherwise move up to 25 pounds.

Responsibilities
  • Deliver compassionate assistance to patients in alignment with the organization's Model of Care.
  • Act as an expert resource to medical assistants.
  • Obtain and record patient medical histories, vital signs, test results, and other information for the medical chart while maintaining confidentiality in alignment with HIPAA, HITECH, and California regulations.
  • Prepare patients for visits and provider examinations by performing preliminary screenings, vital signs, measurements, rooming, and charting duties.
  • Observe patients, record and report significant conditions and reactions, and notify a supervisor or medical provider of patient conditions, reactions to drugs or treatments, and significant incidents.
  • Perform chart preparation and confirmation calls before scheduled appointments.
  • Follow Epic electronic medical record workflows for standard rooming intake, telephone call documentation, refill encounters, and appropriate message inboxes and pools.
  • Conduct medical assistant procedures such as electrocardiography, respiratory testing, vision and hearing tests, ear irrigations, peak flow testing, and nebulizer treatments.
  • Administer medications and vaccines as ordered by a medical provider or protocol using the seven rights of administration.
  • Set up, assist with, and clean up after minor clinic procedures, including Pap smears, mole removals, minor lacerations, and incision and drainage procedures.
  • Obtain specimens and perform point-of-care testing according to laboratory protocols.
  • Stock and clean examination rooms to remove clutter and ensure patient safety.
  • Dispose of biohazardous waste properly.
  • Use standard medical asepsis practices to clean and disinfect examination rooms and other patient-facing clinic areas.
  • Ensure proper sterilization and packaging of instruments and maintain the autoclave machine.
  • Follow universal precautions, including handwashing and use of personal protective equipment as appropriate.
  • Schedule appointments and follow-up visits according to health center procedures.
  • Explain procedures and treatments to patients under the direction of a registered nurse or medical provider.
  • Identify workflow and process improvements and assist clinic leadership with testing and implementing improvements.
  • Float to other clinics when needed to provide coverage or meet minimum staffing levels.
  • Support delivery of medications to patients' homes when necessary.
  • Assist in chronic care management, including outreach calls and care coordination.
  • Assist with or facilitate routine daily team huddles.
  • Participate in evaluation of peers and support staff.
  • Participate in required staff meetings.
  • Participate in Open Door quality improvement activities, meetings, and initiatives.
  • Perform specialized duties, projects, and other duties assigned by the supervisor.
  • Act as a clinical supervisor for medical assistants on the Care Team as needed.
