Requirements
  • A Master's Degree or commensurate experience and satisfactory completion of Nurse Practitioner licensure.
  • Current Nurse Practitioner licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
  • Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current Basic Life Support, Advanced Cardiovascular Life Support, or Cardiopulmonary Resuscitation certification.
  • A valid state driver's license for the state of residence.
  • Automobile insurance coverage required for the state of residence, or the ability to obtain it, including $100,000 bodily injury coverage per person, $300,000 per incident, $50,000 property damage coverage, and collision and comprehensive deductibles of $1,000 or less.
  • Strong computer skills and familiarity with employee health or medical record software.
  • Ability to work with members in their homes or skilled nursing facilities.
Responsibilities
  • Conduct adult and geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Administer and order point-of-care and laboratory-based testing as needed.
  • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with the primary care physician on patient education and follow-up as necessary.
  • Provide services at members' places of residence, including homes and skilled nursing facilities.
  • Complete scheduled health assessments during available daytime, evening, and weekend hours.
  • Travel to see members outside the home location when additional availability is offered.
  • Complete onboarding training and participate in preceptor support, one-to-one feedback, and continuing education.
Desired Qualifications
  • One year of Nurse Practitioner experience is preferred.
  • New graduates are encouraged to apply.
  • Additional state licensure and credentialing in neighboring states or other states may be supported when business need exists.
