Requirements
  • A current pharmacist license issued by the Oregon State Board of Pharmacy.
  • The pharmacist must be available to be on call on weekdays and weekends.
Responsibilities
  • Identify, resolve, and document complex therapeutic problems for the home infusion patient population.
  • Monitor therapeutic response and adverse effects of medications to guide home infusion drug therapy, consult providers when questions or problems arise, and report adverse drug reactions, drug defects, and medication errors.
  • Respond to drug-information requests from physicians, nurses, patients, and patients' families; independently review and interpret literature and apply it to specific patient-care situations.
  • Interpret and evaluate physicians' orders and prescriptions for accuracy, completeness, and appropriateness.
  • Verify the accuracy and appropriateness of medications prepared and dispensed from the home infusion pharmacy.
  • Supervise pharmacy technicians' activities and assume responsibility for duties performed by pharmacy technicians in the work area.
  • Provide medication-therapy recommendations and make independent decisions about home infusion medication therapy in cooperation with healthcare team members.
  • Coordinate medication delivery and care with healthcare team members.
  • Educate patients about their medications and collaborate with inpatient and outpatient interdisciplinary teams.
  • Teach and precept students, interns, and/or residents.
Desired Qualifications
  • Training or experience in a specialized area at the time of recruitment.
  • PGY-2 specialty residency.
  • Basic Life Support, Advanced Cardiovascular Life Support, or Pediatric Advanced Life Support certification.
  • Vaccination certification.
  • Specialist certifications such as Board Certified Pharmacotherapy Specialist or Board Certified Oncology Pharmacy Specialist.
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy plus a practice residency or two years of work at an infusion pharmacy location.
  • Ambulatory or outpatient infusion pharmacy practice experience.
  • Ability to enforce pharmaceutical-care principles safely and accurately.
  • Ability to make age-related and disease-related modifications to drug therapy.
  • Ability to evaluate medical literature.
  • Ability to work independently with minimal direct supervision and cooperatively with health-system and pharmacy staff.
  • Ability to communicate effectively and professionally orally and in writing.
  • Ability to perform accurately with attention to detail, exercise sound professional judgment, and perform systematic problem solving.
  • Ability to enforce practice guidelines, standards, and medication-related policies and make critical decisions.
  • Computer skills including use of the electronic medical record, word-processing and spreadsheet software, medical literature, and medication databases.
