RN/RPN degree or diploma from a recognized college or university and CNO registration in good standing.
Experience in clinical skills, assessing client care needs, and formulating care plans.
Experience using technology tools, including electronic documentation systems such as Goldcare.
Experience developing clear and concise reports.
Knowledge of the Occupational Health and Safety Act, relevant government legislation including the Employment Standards Act, and the Ontario Human Rights Code.
A valid driver's license and use of a car on the job are required.
Availability to participate in VHA's on-call roster.
Responsibilities
Conduct an initial visit and complete a thorough assessment to establish a care plan according to client needs.
Ensure initial and interim home visits, as well as clients' concerns and complaints, are addressed and documented.
Provide teaching on special functions and delegation in accordance with College of Nurses standards.
Collaborate with clients, families, and other care team members to establish a plan of care.
Advise and support clients and their families at home using a holistic approach to care.
Provide after-hours/on-call support, using clinical judgment to visit clients at home when required.
Maintain documentation standards according to the College of Nurses of Ontario and VHA policies.
Support quality-improvement projects and process improvement.

The Care Team Clinical Lead (CTCL) is a crucial role in promoting and maintaining a positive overall client and family experience and ensuring safe and effective care in compliance with VHA’s policies, Ontario legislation, and standard of practice. The primary role of the CTCL is to conduct in-home client visits to assess, develop, and monitor Client Care Plans; provide clinical support, education, and training to personal support workers for delegated acts as well as special functions. Follows up with PSWs to ensure their skills and competencies are up to date and monitor clinical documentation; liaises with Ontario Health atHome. The CTCL takes an active role in clinical focused committees and quality improvement projects.

Key Areas of Accountability
VHA's Care Team Clinical Lead is required to do the following tasks, but not limited to:
