Responsibilities
  • Maintain the security, confidentiality, completeness, and accuracy of medical records in accordance with organizational policies, procedures, and regulatory guidelines.
  • Act as the hospital Privacy Officer when assigned.
  • Oversee compliance efforts related to the Centers for Medicare & Medicaid Services Review Choice Demonstration and Final Rule Audit.
  • Serve as the primary on-site contact for Review Choice Demonstration and Final Rule Audit compliance initiatives.
  • Direct, plan, schedule, and participate in day-to-day Health Information Management department activities, including indexing, transcription, quantitative analysis, chart completion, release of medical record information, and abstracting medical information.
  • Oversee concurrent medical record completion and collaborate across disciplines to ensure accuracy and Final Rule adherence.
  • Act as the Cerner superuser and subject-matter expert for auditing Final Rule elements, and support providers using Cerner.
  • Direct record assembly and review medical records for data elements required for chart completion.
  • Monitor and evaluate physicians and hospital staff for compliance with record-keeping requirements.
  • Oversee the development, implementation, maintenance, and adherence of policies and procedures concerning the privacy of and access to patient health information.
  • Collaborate with Review Choice Demonstration leadership and hospital staff on process improvement and education regarding documentation and timeliness.
  • Provide development guidance and assist with identifying, implementing, and maintaining information privacy policies and procedures with hospital administration, the Corporate Compliance Officer, and legal counsel.
  • Perform initial and ongoing credentialing for hospital medical staff when assigned.
  • Safeguard medical-record confidentiality by ensuring compliance with the Release of Information policy, securing legal and risk-management records, responding to subpoenas and court orders, and representing the hospital in court hearings or depositions when required.
  • Assess safety risks and implement appropriate precautions for patients, visitors, and staff.
  • Comply with approved safety and Infection Prevention standards.
  • Complete assigned training.
  • Follow established protocols to facilitate Medicare affirmations and respond to Review Choice Demonstration non-affirmations once the hospital is formally under the program.
  • Stay informed about changes in Review Choice Demonstration and Final Rule Audit processes, including regional Medicare Administrative Contractor approaches and review outcomes.
  • Communicate admission non-affirmation and denial reasons with hospital and Review Choice Demonstration leadership and assist with necessary justifications.
  • Assist with denials through the appeal process by synthesizing clinical documentation for each patient stay into service justifications for all payors.
  • Manage tracking systems so deadlines are met and real-time data on new admissions is available for timely submissions.
  • Perform other assigned duties supporting organizational effectiveness.
Desired Qualifications
  • Two years of medical coding experience.
  • A Health Information Management program accredited by the Commission on Accreditation for Health Informatics and Information Management.
  • Experience in a management role.
  • Registered Health Information Administrator or Registered Health Information Technician certification.
  • Certified Coding Specialist certification as an additional credential.

ClearSky Health operates inpatient rehabilitation facilities (IRFs) across multiple states, delivering post-acute rehabilitation services funded by Medicare and other insurers. It builds, acquires, and runs hospitals to form an integrated hospital-to-home rehabilitation network, growing through de novo facilities and acquisitions. Each facility uses a physician-led interdisciplinary team, provides at least 15 hours of therapy per week, 24/7 rehabilitative nursing, and a full range of therapies, along with home-like spaces and case management. ClearSky Health announced plans to build two new 36-bed medical rehabilitation hospitals in Casa Grande, Arizona, and Seneca, South Carolina. The facilities will expand the organisation to 19 hospitals nationwide.
