Requirements
  • At least one year of experience in healthcare, case management, homeless services, working with vulnerable populations, or a related field.
  • Employment eligibility verification is required.
  • Have or be able and willing to obtain CPR/First Aid training.
  • Successfully complete a tuberculosis test, background screening, and drug test as a condition of hire.
  • Have reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
Responsibilities
  • Work with case managers in interim housing settings to support the integration of Problem-Solving in their cases.
  • Assist with training all Family Solution Center staff on Problem-Solving.
  • Work with the LAHSA Problem-Solving Unit to streamline PSAF requests and intervention updates.
  • Ensure Problem-Solving tracking is completed across the Family Solution Center.
  • Work with key partner family systems to integrate Problem-Solving upstream from the Coordinated Entry System and reduce inflow.
  • Process and accept referrals from funders.
  • Meet with assigned guests at least once per week to support progress toward goals identified in their Housing Service Plans.
  • In collaboration with a supervisor or MSW/LCSW staff, conduct intakes and individualized needs assessments for guests.
  • Collaborate with guests to develop Individualized Service Plans addressing barriers to services, housing, and appropriate healthcare.
  • Attend and participate in assigned trainings and learning collaboratives.
  • Facilitate psychosocial rehabilitation groups for guests as required.
  • Conduct a full Vulnerability Index–Service Prioritization Decision Assistance Tool assessment at admission for all new guests to support Individualized Service Plan development.
  • Complete new guest intakes and enter information into the Homeless Management Information System in a timely manner.
  • Provide ongoing case-management support with benefits establishment, obtaining identification and vital documents, employment linkages and assistance, housing referrals, and health-related assistance identified in the Individualized Service Plan.
