Requirements
  • A bachelor's degree in an appropriate human services field and one year of experience providing direct services to people with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse, or experience effectively linking people with services that address social determinants of health is required, or an equivalent combination of education and experience.
  • A valid in-state driver's license with a satisfactory driving record and access to an automobile upon hire are required.
Responsibilities
  • Provide professional comprehensive care management services to patients of specific locations and/or programs.
  • Collaborate with health, behavioral health, and social service providers and assess patients' needs, develop and manage care plans with patients enrolled in care management.
  • Serve complex, high-utilizing patients who need comprehensive care management services, including care coordination, health promotion, comprehensive transitional care, enrollee and family support, referral to community and social supports, and use of technology to link services.
  • Develop comprehensive care management plans that highlight and support patient goals, objectives, and interventions intended to increase self-efficacy and engagement with community providers, using person-centered practices.
  • Interact with patients through telephonic outreach and in-person encounters in primary care settings, behavioral health clinics, homes, jails, hospitals, homeless shelters, and other community settings; conduct assessments to identify service needs.
  • Develop, review, and discuss plans with patients and care teams, focusing on linking individuals to clinical and social services with system and community providers.
  • Utilize community and family resources to create sustainable support systems for patients.
  • Perform complex care management services consistent with University of Rochester Medical Center and New York State regulations and policies for Health Home Services.
  • Establish and maintain cooperative working relationships with community providers to obtain needed services and support for enrolled patients.
  • Coordinate outreach and engagement activities focused on finding, connecting, and retaining patients in Health Home Care Management Services.
  • Complete timely and thorough documentation of services in electronic medical records in compliance with hospital policies and Health Home regulations; assist with record reviews and quality initiatives.
  • Monitor utilization of services and encourage enrollees to follow treatment recommendations; ensure care is accessible, attended, and effective.
  • Partner with patients and community providers to reduce unnecessary emergency and inpatient services; support transitions of care, appointment adherence, and barrier resolution.
  • Support population health initiatives.
  • Perform other duties as assigned.
