Renal Care Coordinator
About the job
- A minimum of two years of previous experience in clinical renal patient care is required.
- Experience in renal transplant, dialysis, or chronic kidney disease patient care is required.
- An understanding of diabetes and cardiovascular disease processes is preferred.
- Strong organizational and communication skills are required.
- Assess patients' knowledge of late-stage chronic kidney disease and treatments, and educate and inform them to support informed decisions during the transition to renal replacement therapy.
- Provide support, guidance, and care coordination for patients seeking conservative or palliative care.
- Act as a liaison with appropriate staff to ensure patients and family members receive comprehensive information about modality advantages and disadvantages; home and in-center hemodialysis; peritoneal dialysis; kidney transplantation; conservative care; and hemodialysis access types, emphasizing the health and safety benefits of arteriovenous fistulas or arteriovenous grafts compared with central venous catheters.
- Organize the nephrology practice's late-stage chronic kidney disease patient population and education, including modality selection, permanent access placement and maintenance, and a stable transition to renal replacement therapy.
- Participate in interpreting summary clinical data and using it to improve late-stage chronic kidney disease care processes.
- An understanding of diabetes and cardiovascular disease processes.
