About the job
- An active New Jersey Registered Nurse (RN) license is required.
- A minimum of 2 years of recent RN experience in acute care, home care, or case management is required.
- Recent experience with ventilator and tracheostomy care, G-tube/PEG tube management, IV infusions, and wound care is required.
- Current BLS certification is required.
- A current physical examination within the past year is required.
- A current PPD or QuantiFERON test within the past year, or a chest X-ray completed within the past five years if applicable, is required.
- MMR immunization documentation is required.
- A valid driver's license and reliable transportation are required.
- Authorization to work in the United States is required.
- Conduct comprehensive in-home nursing assessments.
- Develop, implement, and oversee individualized plans of care.
- Obtain and maintain physician orders in compliance with New Jersey regulations.
- Monitor client conditions and promptly communicate significant changes.
- Complete accurate and timely clinical documentation.
- Perform Home Health Aide supervisory visits and ensure quality of care.
- Provide clinical guidance and support to Home Health Aides and Nurses.
- Promote client safety, independence, comfort, and overall well-being.
