Pair Team, an AI-enabled medical group for Medicare and Medicaid beneficiaries, has acquired Town Square, a social care coordination company. Financial terms were not disclosed. The acquisition aims to integrate clinical, behavioural and social care through an AI-driven model that coordinates complex care whilst human teams focus on clinical judgement and relationship-driven support. Town Square's platform connects health plans, providers and community-based organisations to manage social determinants of health at scale. Royal Tuthill, Town Square's CEO and founder, will join Pair Team as senior vice president of networks, leading national expansion of Medicare programmes and deepening community partnerships.
