WelbeHealth provides comprehensive care specifically designed for older adults, focusing on improving their quality of life and maintaining their independence. The company uses the PACE (Program of All-Inclusive Care for the Elderly) model, which integrates medical, social, and long-term care services for seniors who are eligible for nursing home care but prefer to remain at home. Each patient receives an individualized care plan that is tailored to their unique needs and preferences, developed in collaboration with them and their families. WelbeHealth offers a wide range of services, including medical care, physical therapy, social activities, and transportation, funded through a mix of government programs like Medicare and Medicaid, as well as private payments. The leadership team has extensive experience in geriatric care and the PACE model, setting WelbeHealth apart from competitors by emphasizing personalized, patient-centered care for the elderly.
