Requirements
  • An unencumbered Licensed Practical Nurse license in the state of employment.
  • Current cardiopulmonary resuscitation and Basic Life Support certification.
  • Average to advanced computer and software skills, including Microsoft Word, Excel, and Outlook.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation.
  • Proof of liability and property damage insurance for the vehicle used.
  • If driving a personal vehicle, a driver’s license held for at least five years, no disqualifying driving or criminal history described in the posting, no more than two moving violations or one at-fault accident in the last three years, and no more than three moving violations within the last five years.
  • Full vaccination for frontline workers, subject to medical or religious exemption review.
Responsibilities
  • Partner with team members to develop and implement clinical treatment plans under the clinical supervision of providers and registered nurses and the direction of team members, including active participation in team meetings.
  • Support wrap-around clinical services in home and field settings through co-visits, assisted telehealth visits, and independent Licensed Practical Nurse visits.
  • Provide clinical and clinical coordination services according to patient clinical risk assessments and care plans under the supervision of providers, Practice Managers, and Registered Nurse Case Managers.
  • Conduct health screenings for newly enrolled patients and on an ongoing basis.
  • Perform field clinical services, including wound care dressing changes, phlebotomy, medication review, and patient status checks.
  • Meet with unreachable patients in their homes to re-engage them in the program.
  • Provide cross-coverage for planned and unplanned team-member absences to prevent interruptions in patient care.
  • Partner with interdisciplinary team members to improve quality of care, reduce cost of care, and improve patient and staff satisfaction.

  • Experience providing whole-person care, including care for patients living with mental illness and substance use disorders.

It serves Medicare, Medicaid, and Medicare Advantage beneficiaries with complex needs, coordinating with an existing primary care provider when available.
