Requirements
  • Maintain a current, unencumbered Pennsylvania nursing license in good standing with the State Board of Nursing.
  • Maintain all required continuing education.
  • Be at least 18 years of age.
  • Pass a Clear Cisive background check reviewing the previous seven years.
  • Hold active CPR certification; Basic Life Support certification with skills is preferred.
  • Meet the facility’s general health requirements, including a physical examination and a two-step purified protein derivative test or equivalent.
  • Be able to sit, stand, walk, talk, read, and hear while performing the job duties.
  • Be able to push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry or move that weight at least 50 feet.
  • Read, write, speak, and understand English, including reading technical procedures and comprehending policy and procedure manuals.
  • Effectively present information and respond to questions from managers and employees and communicate information about residents’ conditions.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by certified nursing assistants.
  • Work under the supervision of the Director of Nursing and Assistant Director of Nursing to ensure compliance with state, federal, and local standards.
  • Participate in developing, maintaining, and implementing the nursing service quality assurance program and participate in authorized facility surveys and inspections.
  • Admit, transfer, and discharge residents as required.
  • Greet newly admitted residents, escort them to their rooms, and orient residents and family members to the facility.
  • Make rounds with physicians as necessary and arrange diagnostic and therapeutic services ordered by physicians.
  • Consult with residents’ physicians regarding care, treatment, rehabilitation, and related needs.
  • Review resident charts for treatments, medication orders, diets, and other required information.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Complete and file records and charts for resident admissions, transfers, and discharges.
  • Document nursing notes and routine charting in an informative, descriptive, and timely manner, sign and date medical-record entries, and reflect care provided and resident responses.
  • Review resident care plans, ensure they are followed, and communicate care plans to assigned certified nursing assistants.
  • Review and update written discharge plans and assist with the nursing-services portion of discharge planning.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Complete, submit, and chart accident and incident reports according to established procedures.
  • Inform family members of resident deaths, contact funeral homes when requested, and ensure postmortem procedures are followed.
  • Prepare resident charge slips, maintain the daily census report, and submit required information to the Business Office.
  • Complete medical forms, reports, evaluations, studies, charting, and other administrative duties.
  • Protect assigned user IDs, passwords, and resident protected health information and report suspected unauthorized access or disclosure.
  • Encourage attending physicians to review and sign treatment plans, orders, and progress notes.
  • Receive telephone orders from physicians and transcribe them to charts, medication cards, treatment plans, and care plans.
  • Ensure personnel providing direct care follow resident care plans and residents’ wishes.
  • Check residents who cannot call for help, meet with residents and family members, and report problem areas to the Nurse Supervisor.
  • Arrange transportation for discharged residents and ensure discharged residents are escorted to the pickup area.
  • Prepare and administer medications as ordered, verify resident identity, prevent medication errors, and maintain narcotic records.
  • Implement nursing objectives and standards, check that treatments are properly administered, and evaluate residents’ physical and emotional status.
  • Record diet orders, forward changes to Food Services, report discrepancies, and complete transfer forms.
  • Document unauthorized discharges in residents’ medical records.
  • Maintain adequate stocks of medications, supplies, and equipment and order them as needed.
  • Review medication cards, monitor physician-order transcription and stop orders, notify physicians about automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend dismissals or transfers to the Nurse Supervisor.
  • Report staffing needs and absences to the Nurse Supervisor or Scheduler.
  • Review the department workforce and make staffing recommendations.
  • Develop work assignments, assist with assignments, and provide leadership to nursing personnel on the assigned unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards and report problem areas.
  • Ensure department personnel, residents, and visitors follow established policies and procedures.
  • Maintain working relationships with interdisciplinary and other facility departments.
  • Create and maintain a calm, respectful, and positive environment throughout the unit and shift.
  • Administer disciplinary action fairly and report required information to the Nurse Supervisor.
  • Report occupational exposures, fraud, information-system access violations, and other incidents according to facility procedures.
  • Inform nursing personnel about new admissions and ensure rooms are ready.
  • Notify physicians and next of kin about accidents, incidents, or changes in residents’ conditions.
  • Carry out restorative and rehabilitative programs, including self-help and care activities.
  • Inspect treatment areas and ensure they are clean and safe.
  • Perform nursing procedures including catheterization, tube feeding, suctioning, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care for dying residents.
  • Use restraints when necessary and according to policy.
  • Obtain specimens for laboratory testing and record temperature, pulse, respirations, and blood pressure; monitor seriously ill residents.
  • Develop, plan, conduct, and schedule in-service training and maintain an effective new-employee orientation program.
  • Train personnel on tasks involving potential blood or body-fluid exposure and assist with clinical supervision planning for nurse aide trainees.
  • Attend facility, outside, continuing-education, and advance-directive training programs.
  • Monitor assigned personnel’s compliance with safety regulations, infection-control procedures, hand hygiene, personal protective equipment, and emergency drills.
  • Maintain clean, safe, and sanitary nursing areas and ensure required safety equipment is used.
  • Participate in infection-control programs for communicable and infectious diseases.
  • Recommend unit equipment and supply needs and maintain adequate medication, supply, and equipment stock levels.
  • Develop and implement procedures for safe nursing-equipment operation and ensure only trained, authorized personnel operate equipment.
  • Monitor nursing procedures to use supplies efficiently and avoid waste.
  • Assist with MDS and RAP planning, scheduling, and revision.
  • Provide data to the Quality Assurance and Assessment Committee.
  • Review department policies, procedure manuals, and job descriptions; meet with nursing and support staff to plan shift services; maintain the Nursing Service Procedures Manual; and ensure nursing personnel comply with it.
