Graduate of an accredited Physical Therapy Program with a diploma, Master of Science, or Bachelor of Science degree.
Hold a license to practice, or be eligible to practice, in the State of Maine.
Hold a valid Maine driver's license.
Hold CPR/Basic Life Support certification.
Responsibilities
Perform physical therapy evaluations and reassessments per physician referral to assess patients’ physical, functional, social, and emotional needs.
Interpret evaluation findings and diagnostic information to develop individualized treatment plans and functional goals designed to restore, maintain, and improve patient mobility and functional performance.
Implement physical therapy interventions based on the established plan of care and modify treatment as indicated by patient response, performance, physician input, and updated diagnostic information.
Assist in discharge planning, including development of home programs, discharge evaluations, and coordination of appropriate follow-up care.
Provide ongoing education to patients and families regarding treatment plans, goals, safety, and home management strategies.
Communicate patient status, treatment progress, and care activities with members of the interdisciplinary healthcare team and participate in interdisciplinary meetings, utilization review, and family conferences as appropriate.
Complete accurate and timely documentation of patient care activities in the medical record, including evaluations, progress notes, reassessments, discharge summaries, and required billing documentation, and ensure patient charges are completed appropriately.
Use clinical reasoning and professional judgment to prioritize patient care, resolve clinical issues, and collaborate with physicians and other healthcare professionals when consultation is needed.
Participate in professional development and departmental activities, including in-service attendance, assigned educational presentations, community or industrial education programs, and clinical instruction or job-shadow mentoring when requested.
Support departmental operations and program development, including assisting with new program implementation, maintaining productive use of work time, demonstrating cost-consciousness, and collaborating with hospital staff and departments.
Comply with hospital and departmental policies, procedures, and regulatory requirements.
Participate in the hospital-wide Quality Improvement Program and support and implement department-specific quality-improvement initiatives and projects.
Recommend process improvements, report service-delivery quality issues, and maintain a focus on quality improvement and organizational excellence.
Participate in emergency and disaster preparedness planning and drills as requested and participate in the hospital’s response to a real-life disaster or emergency event within the scope of professional and personal ability.

Position: Full Time, salaried, non-union, exempt.
SIGN ON BONUS: Amount would be $7,500. 25% paid after the probationary period is completed. 25% to be paid at
6 months and 50% to be paid after the first year of employment.

Disaster Management: As an employee of Penobscot Valley Hospital, the position has an inherent role to care for our community members when in need. To this regard, the incumbent will be expected to participate in emergency/disaster preparedness plan participate in emergency/disaster preparedness planning and drills as requested. as requested. as requested. Requirements
