Requirements
  • Active, unencumbered license to practice nursing.
  • ANCC or AANP board certification as a Nurse Practitioner in Family, Adult, Gerontology, or Emergency Medicine.
  • Current CPR certification.
  • Compliance with required healthcare personnel vaccinations and preventive testing, including Hepatitis B, influenza, MMR, Tdap, varicella, and tuberculosis testing.
  • Ability to effectively communicate with elderly and chronically ill patients and their families.
  • Ability to practice autonomously in a remote clinical environment, including independently conducting patient assessments, formulating evidence-based treatment plans, managing complex chronic conditions, and making sound clinical decisions without direct on-site supervision.
  • Proficiency with computer platforms, electronic health records, secure messaging systems, and cloud-based documentation tools.
Responsibilities
  • Complete comprehensive, accurate, and thorough reviews of assigned members, including timely completion and submission of required encounter documentation.
  • Document all pertinent and active medical conditions in the medical record in compliance with CMS/DHHS, company policy, and client requirements.
  • Support the physician-patient relationship through timely communication and documentation of assessment findings, recommendations, additional service needs, emergency service needs, and follow-up requirements.
  • Provide general recommendations to members to improve their knowledge of chronic conditions, including recommended testing.
  • Address and close identified disease-specific or preventive gaps in care.
  • Recognize emergent or urgent situations requiring escalation and take appropriate action according to company policies and professional judgment.
  • Perform, document, and communicate results of point-of-care testing.
  • Maintain compliance with company policies, procedures, confidentiality requirements, and HIPAA requirements.
  • Provide at least 25 availability slots per month, approximately 37 hours.
  • Perform other duties as assigned.
Desired Qualifications
  • Home health experience.
  • Understanding of Medicare, Medicaid, and health plan benefit structures.
  • Bilingual or multilingual ability.
