Requirements
  • Current, active, unrestricted, and unencumbered Ohio Registered Nurse license or qualifying multistate compact Registered Nurse license, as applicable to the service location.
  • At least 3 years of clinical nursing experience working with older adults and/or individuals with chronic or complex medical conditions.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently and exercise appropriate professional and clinical judgment within Registered Nurse scope of practice.
  • Ability to engage effectively with older adults, authorized representatives, facility staff, and healthcare professionals.
  • Strong organization and time-management skills with the ability to coordinate accepted services across multiple members and facilities.
  • Comfort using electronic clinical documentation systems, mobile devices, and telehealth technology.
  • Reliable transportation and ability to travel to long-term care facilities within the designated service area.
  • Successful completion of applicable background screening, healthcare screening, exclusion screening, credentialing, privileging, and other requirements necessary to qualify for the engagement.
  • Maintain all licenses and professional credentials required to perform accepted services in good standing throughout the engagement.
  • Cooperate with applicable credentialing, privileging, payer enrollment, recredentialing, and related requirements.
  • Complete required privacy, security, compliance, patient-safety, technology, and clinical-system orientation.
  • Use Avail Health-managed devices, accounts, applications, or systems when required to protect protected health information or confidential information and satisfy applicable requirements.
Responsibilities
  • Independently conduct in-person Health Risk Assessments with Institutional Special Needs Plan Medicare Advantage members residing in long-term care facilities.
  • Complete comprehensive assessments covering medical, functional, cognitive, behavioral health, psychosocial, and other required domains.
  • Document assessment findings to support development and ongoing maintenance of member-driven Individualized Care Plans.
  • Engage members regarding their health needs, goals, preferences, and priorities.
  • Engage authorized representatives when their participation is needed for assessment or care-planning activities.
  • Coordinate with facility staff and interdisciplinary care team members to obtain relevant information and support assessment and care-planning activities.
  • Identify potential changes in condition, care gaps, or unmet needs and communicate findings through clinical escalation and care coordination processes.
  • Complete required Health Risk Assessment and supporting clinical documentation within applicable timeframes.
  • Partner with Nurse Practitioners to support Chronic Disease Assessments for Institutional Special Needs Plan members.
  • Obtain required clinical information, vital signs, and designated screening information.
  • Facilitate virtual Nurse Practitioner encounters, including technology setup and real-time coordination among the member, facility team, and Nurse Practitioner.
  • Provide on-site clinical support during virtual encounters, including physical assessment components within Registered Nurse scope of practice and applicable protocols.
  • Communicate relevant clinical findings and follow-up needs to Nurse Practitioners and other care team members.
  • Complete applicable clinical documentation and support timely completion of accepted Chronic Disease Assessment services.
  • Provide services at long-term care facilities within the designated service area for accepted assignments.
  • Coordinate accepted member and facility assignments and complete associated services and documentation.
  • Establish working relationships with members, authorized representatives, facility staff, Nurse Practitioners, and interdisciplinary care team members.
  • Maintain member privacy and comply with applicable privacy, security, payer, facility, credentialing, clinical-quality, and patient-safety requirements.
  • Use approved technology, applications, and equipment for secure access to clinical information and completion of services.
  • Fulfill the clinical, documentation, continuity, and contractual responsibilities associated with accepted services.
Desired Qualifications
  • Long-term care, skilled nursing, geriatric, or post-acute care experience.
  • Experience conducting Health Risk Assessments or other comprehensive nursing assessments.
  • Medicare Advantage or Institutional Special Needs Plan experience.
  • Experience with individualized care planning or interdisciplinary care management.
  • Risk adjustment or chronic disease assessment experience.
  • Prior mobile, field-based, or multi-facility nursing experience.
