Requirements
  • Two years of management or supervisory experience in a rehabilitation therapy department or program; home care experience is preferred.
  • A degree from a baccalaureate or master's program in an applicable discipline approved by an accredited organization.
  • A current California Physical Therapist license.
  • Current Basic Life Support certification.
  • A valid California driver's license with an acceptable driving record.
  • All licenses and certifications must be current at hire and maintained throughout employment.
Responsibilities
  • Provide in-field oversight of clinicians who provide care and treatment to patients in their residences.
  • Handle incoming calls promptly and follow up as needed to meet internal and external customer needs.
  • Coordinate training and educational programs for field nurses to improve clinical outcomes and competencies.
  • Assess staff learning needs and formulate specific plans and goals for the next evaluation period with employees; perform administrative call duties; and serve as a role model and resource for staff.
  • Serve as the first point of contact for field clinicians and assist with patient-care escalations.
  • Coordinate at least biweekly case conferences with the clinical team to discuss patient progress and revise care plans as needed to meet planned outcomes.
  • Review and evaluate each case by reviewing clinician services, conferences, and records; discuss and verify impressions; instruct and guide clinicians to promote effective performance and quality home-care delivery; and remain available during operating hours to assist clinicians as appropriate.
  • Review patients' clinical diagnoses, medications, procedures, and clinical courses.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
