Requirements
  • Registration with the Nova Scotia College of Nursing as a Licensed Practical Nurse in good standing is required.
  • A valid driver's license and access to a vehicle are required.
  • Candidates must be reliable and flexible.
  • Candidates must be willing to travel to and from patient residences within a designated area.
  • Experience working with vulnerable populations is required.
  • Required on-call coverage includes after-hours availability to ensure patient safety and meet patient care needs.
Responsibilities
  • Participate in scheduled community visits to address patient needs within the scope of practice.
  • Provide ethical and safe nursing care.
  • Use appropriate assessment tools and techniques to assess patients' individual needs.
  • Execute and continuously monitor care plans and adjust them as required.
  • Engage patients using a collaborative approach by building rapport and trust.
  • Maintain and update professional patient records related to patient encounters and care plans.
