  • An Associate's degree in human services and two years of full-time work experience providing care or education to vulnerable adults or children, or a Bachelor's degree in human services and one year of full-time work experience providing care or education to vulnerable adults or children.

Responsibilities
  • Coordinate client care, including establishing quarterly case conferences with the case manager, treatment team, family members, or other people the client wishes to attend; contact residents’ family members, therapists, or case managers as needed.
  • Develop, implement, and review quarterly the Individual Service Plan, Individual Abuse Prevention Plan, Emergency Service Plan, and relevant behavior plans in consultation with the Program Manager or Program Director.
  • Coordinate and schedule client medical appointments, complete referrals, and arrange transportation.
  • Identify individuals prone to behavior emergencies and develop appropriate programming for their needs, including impulse control, histories of dangerous behaviors, and self-injurious behaviors; serve clients who typically have higher service needs and more acute symptoms than foster care clients in MHC-I houses.
  • Assist the Program Director and Program Manager with client admission and discharge processes, including gathering clinical information, developing clinically appropriate treatment plans, and connecting clients with community resources upon discharge.
  • Orient new residents to RSS.
  • Help defuse and de-escalate potential behavior emergencies, and use crisis assessment tools for suicidal and aggressive behavior.
  • Assist clients with independent living skills, including hygiene checks, housekeeping, and budgeting.
  • Assist residents with budgeting and coordinate budgets with representative payees.
  • Help residents complete financial forms, including benefit applications upon admission.
  • Ensure facility security and provide general supervision of residents, including creating staff schedules; follow RADIAS Health RSS policies and procedures.
  • Secure coverage for missed shifts, planned or unplanned, including covering shifts when no other coverage is secured.
  • Complete paperwork, reports, and charting in a timely and organized manner.
  • Lead weekly house staff meetings and distribute meeting minutes to the Program Director, Program Manager, Program Nurse, and other house Leads before the Lead Meeting.
  • Attend and participate in weekly Lead Meetings.
  • Administer medications according to RSS policies and procedures under the facility nurse’s supervision.
  • Ensure completion of monthly pharmacy medication orders and ensure clients have all ordered medications.
  • Update medication sheets when medication changes occur.
  • Produce and distribute incident reports during the relevant shift, as needed.
  • Assist with licensing requirements and inspections, including posting emergency procedures.
  • Perform client record audits for compliance with DHS and county RSS regulations, as needed.
  • Ensure fire and tornado drills are completed monthly.
  • Document financial expenditures in QuickBooks and manage petty cash and budgets.
  • Rotate the Emergency Shift Coverage pager.
  • Perform routine cleaning and light house maintenance, and request maintenance assistance as needed.
  • Prepare, serve, and clean up meals according to Health Department standards.
  • Answer the staff telephone and read and respond to company emails during every shift.
  • Oversee planning and coordination of recreational and social activities.
  • Oversee planning of holiday activities and special events.
  • Transport clients in the community as needed.
