Requirements
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
  • Before the start date, as applicable to job location, hold a current unrestricted pharmacist license in Massachusetts, or a current unrestricted pharmacist license or 90-day temporary pharmacist license in Rhode Island; temporary license holders must obtain full Rhode Island licensure before it expires and maintain current unrestricted licensure.
  • Within six months of the start date, for positions performing immunizations or medication administrations under Rhode Island or Massachusetts pharmacy rules, hold and maintain an American Council for Pharmacy Education-approved Immunization Certification and American Heart Association Basic Life Support Certification.
  • Successfully complete a Post Graduate Year 2 residency from an accredited program in the area of specialty and obtain national board certification in that specialty within one year of hire. Two years of clinical experience, knowledge, and development may substitute for formal training; in that case, obtain national board certification in the specialty or Pharmacotherapy within one year of meeting board certification eligibility requirements.
  • Have effective interpersonal and communication skills, written and verbal, to interact appropriately with colleagues and others.
  • Have effective organizational and presentation skills.
Responsibilities
  • Coordinate and oversee new and established pharmacy clinical services within scope of practice, establishing priorities for effective patient care and clinical pharmacy services under departmental and organizational policies.
  • Provide patient care and education through comprehensive medication reconciliations, medication-related counseling, plan-of-care adherence counseling and support, and general health education coaching and support.
  • Promote safe and effective medication use, identify potential drug interactions, and perform related activities.
  • Facilitate resources to maximize patients’ medication therapy success, considering social determinants of health.
  • Coach professional staff, including the home medical team, practitioners, and providers, on pharmaceutical care and pharmacoeconomic principles to support safe and cost-effective medication use within the integrated care delivery model.
  • Support patients enrolled in the Brown University Health Specialty Drug Patient Management Program and patients covered by affiliated accountable care organizations, in accordance with each program’s policies and procedures.
  • Document patient encounters, interventions, and plans of care in the electronic health record.
  • Participate in developing, implementing, and integrating patient-focused medication therapy management and pharmaceutical care services; review and analyze proposed clinical services and recommend and implement changes, resource allocations, and technologies to improve efficiency and pharmaceutical care.
  • Implement, interpret, and consult on policies, procedures, and standards of practice related to medication use.
  • When practicing under a collaborative practice agreement, follow the services specified in the agreement and ensure compliance with state and federal regulations.
  • Ensure pharmacy clinical services and programs meet or exceed patient-care quality standards and standards of care, accounting for the developmental stages of the patient populations served.
  • Serve as a subject matter expert and real-time resource in the area of specialty.
  • Maintain professional competence in the specialty, including knowledge of new medications, disease-management standards, patient-care practices, and technology, through independent study, professional organization participation, continuing education, and training or retraining as needed.
  • Communicate accurately, verbally and in writing, with staff, management, and customers to facilitate timely drug-information flow.
  • Work with the Pharmacy and Therapeutics Committee to establish drug-therapy management and collaborative-practice protocols; monitor targeted drugs or therapeutic classes and implement corrective strategies to optimize use under clinical guidelines.
  • Participate in quality management, quality improvement, practice performance improvement, and quality-measure activities; identify system improvements and lead or participate in assigned committees, task forces, and performance-improvement teams.
  • Participate in developing the department’s clinical-practice strategic vision and short- and long-term goals in consideration of Brown University Health’s mission and values.
  • Provide input into drug budgeting and resource allocation through pharmacoeconomic principles and methods; monitor drug-utilization projections and recommend corrective strategies.
  • Participate in determining department personnel qualifications and competence and developing tools and standards to improve competency in the specialty; participate in new-employee orientation, in-service training, and continuing education.
  • Educate and train pharmacy residents, pharmacy interns, nurses, physician assistants, medical students, and residents; provide assigned didactic lectures.
  • Precept pharmacy residents and interns, complete competency and evaluation documentation, and oversee pharmacy internship rotations.
  • Present assigned lectures to pharmacists, other health professionals, and the public; support academic affiliations, including serving as adjunct clinical faculty and lecturing for pharmacy, nursing, and physician assistant programs.
  • Conduct pharmacy-practice, clinical, and data-related research aligned with Brown University Health’s mission, goals, and priorities; author research-based abstracts and scholarly papers for publication or presentation under departmental policies.
  • Prepare professional-meeting presentations, submit manuscripts to professional journals, and author articles for the Pharmacy website.
  • Perform immunizations or medication administrations in accordance with applicable Massachusetts or Rhode Island Board of Pharmacy rules and regulations.
  • Perform other duties as assigned.
